Opsonic activity of human ascitic fluid: A potentially important protective mechanism against spontaneous bacterial peritonitis

Abstract: The opsonic activity of 60 ascitic fluids from 47 patients was measured using a standard opsonophagocytic assay. Curve analysis of the opsonic activity compared to the ascitic fluid concentration of total protein, total hemolytic complement, C3 and C4 yielded correlation coefficients of 0.84 (p less than 0.001), 0.84 (p less than 0.001), 0.94 (p less than 0.001) and 0.92 (p less than 0.001), respectively. There appeared to be a threshold of concentration for each protein below which there was no killing of bac… Show more

“…For cirrhotic patients to develop SBP in their ascitic fluid, a translocation of the intestinal bacteria due to portal hypertension should occur (13,18), and a reduction in the ascitic fluid opsonic activity (2,4,19) should occur as indicated by the low-protein concentration in this fluid (2,3,13,20). Since fibronectin is active in the opsonic process (5,21), its involvement in the defense mechanisms protecting against infection has been investigated.…”

“…The present findings confirmed that these groups have different opsonic activities in the ascitic fluid and therefore different risk levels for developing SBP. The higher frequency of tense ascitic fluid in cirrhotic patients with lower levels of A-TP indirectly shows that the dilution factor determines not only the low levels of total protein but also of C3 and C4 in the ascitic fluid (2,18).…”

“…Pelos conhecimentos atuais, sabemos que a atividade opsônica do líquido ascítico (importante elemento de sua defesa) se relaciona diretamente com a proteína na ascite 39 . Desta forma, apenas nas ascites com baixo teor proteico há o risco de desenvolvimento da PBE.…”

“…The presence of portal hypertension gives rise to a significantly lower total protein concentration in the ascitic fluid, compared to patients with peritoneal carcinomatosis without pof tal hypertension (6,8,9). A low protein concentration in the ascitic fluid reflects decreased complement concentrations and decreased opsonic and bactericidal activity (6,10,11). This would explain the susceptibility of malignant ascites to the development of SBP in the presence of portal hypertension.…”
