One-Stage Nipple and Breast Reconstruction Following Areola-Sparing Mastectomy

Abstract: BackgroundSkin-sparing mastectomy with immediate breast reconstruction is increasingly becoming a proven surgical option for early-stage breast cancer patients. Areola-sparing mastectomy (ASM) has also recently become a popular procedure. The purpose of this article is to investigate the reconstructive and aesthetic issues experienced with one-stage nipple and breast reconstruction using ASM.MethodsAmong the patients who underwent mastectomy between March 2008 and March 2010, 5 women with a low probability of … Show more

“…However, a drawback of two-stage reconstruction is that a sufficiently large nipple cannot be made because a small amount of skin is used to create the nipple. Furthermore, in SSM, the portion to be used for the NAC is fixed, and therefore it is expected to be almost unchanged [12]. For these reasons, we performed nipple reconstruction simultaneously with reconstruction with a DIEP flap in patients with SSM and obtained satisfactory results.…”

“…Delay et al [ 15 ] described a similar technique using an LD flap in a smaller cohort with similar postoperative results. Alternative autologous techniques that have been described include the muscle-sparing transverse rectus abdominis muscle (TRAM) flap and the deep inferior epigastric perforator flap [ 16 , 17 ]. The majority of our patient cohort were non-smokers (93.1%) and did not receive adjuvant radiotherapy.…”

“…First, the native breast skin and pectoralis major muscle are preserved and could be used for the implant coverage. Second, the position of new nipple is immediately defined when the skin‐sparing mastectomy is performed through a peri‐areolar incision . Further clinical application would be optimal to investigate the efficacy of our approach.…”
