NSAID is inversely associated with asymptomatic gastric ulcer: Local health examination data from the Korean National Health Insurance Corporation

Abstract: BACKGROUND. Silent peptic ulcer has been considered to be associated with nonsteroidal anti-inflammatory drug (NSAID). The recent studies have reported no relationship between them. AIM. “…Third, although it typically requires an endoscopic confirmation, PUD was determined based on a previous diagnosis by a physician in this study. The incidence of endoscopically confirmed PUD in Korea has been reported to be 2.45–7.0% [ 25 27 ] which was consistent with our results. Also, as the lifetime screening rate for stomach cancer during the study period was about 65%, we believe that the majority of subjects in this study had been diagnosed with PUD by endoscopy.…”

“…PUD requires endoscopic confirmation, which suggests that some of the subjects in our study may not have had PUD. However, the incidence of endoscopically confirmed PUD has been reported to be between 2–10% in Korea, which suggests that our study may not have overestimated PUD incidence34353637. Also, as the lifetime screening rate for stomach cancer during the study period was about 65%, we believe that many of our subjects had been diagnosed with PUD by endoscopy38.…”

“…The main action mechanism of NSAIDs damaging on the stomach and duodenum mucous membrane is its cyclooxygenase (COX) enzyme blockade, which has two isomers: block COX-1 and COX-2 induced [4,5,7]. Inhibition of COX-1 activity, while taking NSAIDs, causes depression of prostaglandin synthesis system, which serves as the main cause of such side effects as gastroduodenal erosive and ulcerative lesions.…”

“…Inhibition of COX-1 activity, while taking NSAIDs, causes depression of prostaglandin synthesis system, which serves as the main cause of such side effects as gastroduodenal erosive and ulcerative lesions. It deplets the stocks in the tissues and causes iatrogenic prostaglandins failure [3,5].…”
