Background
Health care costs are growing faster than the rest of the global economy, according to the World Health Organization (WHO). Countries’ health expenditures include paying for general medicine, diagnostic procedures, hospitalizations and surgeries, as well as medications and prescribed treatment. Primary biliary cholangitis (PBC) is a rare autoimmune liver disease and the first line available treatment is ursodeoxycholic acid (UDCA), however, direct and indirect treatment costs are expensive. Main aim of this trial was to assess if the therapeutic efficacy of UDCA manufactured by the university hospital is equivalent to that of standard UDCA and treatment cost reduction in patients with PBC.
Methods
It is a prospective, interventional, randomized, and crossover study in patients diagnosed with PBC. UDCA 300 mg tablets and capsules were developed and manufactured by the university hospital. Thirty patients under treatment with standard UDCA, in stable doses were randomized in sequence A and B, 15 patients in each arm. The groups were treated for 12 weeks and after, the UDCA formulation was changed, following for another 12 weeks of continuous therapy (tablets and capsules / capsules and tablets). Laboratory tests were performed at time T0 (beginning of treatment), T1 (at the 12 week-therapy, before the crossing-over) and T2 (end of treatment). The evaluation was done by comparing the hepatic parameters ALP, GGT, ALT, AST and total bilirubin, also considering the adverse events. The comparison of costs was based on price of the manufactured UDCA and standard UDCA price of the hospital.
Results
Hospital reduced 66.1% the PBC treatment costs using manufactured UDCA. There were no differences in the biochemical parameters between sequence (A and B) and tablets or capsules of UDCA formulations applied in the treatment of PBC.
Conclusions
The study showed that there was no significant difference between manufactured UDCA (capsule and tablet) and standard UDCA. Hospital reduced the PBC treatment costs using the manufactured UDCA by the university hospital.

Repensar as práticas de exteriorização e divulgação do material escrito arquitetado pelas editoras no cenário contemporâneo da literatura brasileira é de suma importância, principalmente no que concerne à categoria de produção juvenil. Nosso objetivo neste artigo é apresentar, destacar e analisar a trilogia de contos de Murilo Rubião, publicada em 2016 pela Editora Positivo, a partir do aporte teórico do reendereçamento literário, termo cunhado por Vera Teixeira de Aguiar e do fenômeno crossover, estudado por Beckett (2009). Os três livros, vendidos separadamente, são reembalados de maneira a ampliar o público quanto ao acesso da obra do escritor mineiro. Inserida nos moldes da literatura fantástica, tais obras podem atingir públicos de diferentes idades e operar como impulsionador do interesse dos jovens na leitura de uma literatura para adultos, devido ao trabalho de editoração gráfico e ilustrativo proporcionado pelo mercado crossover e pelas maneiras de reendereçamento. Resumo:No discurso mimético de Lygia Fagundes Telles, eventos aparentemente verossímeis cedem lugar à imaginação, imprimindo uma atmosfera de confronto entre o estranho e o familiar, o real e o irreal. No conto "Tigrela", integrante da obra Mistérios (1981), a personagem feminina é delineada com traços que confundem sua identidade. Trata-se da história de Romana e seu animal de estimação, Tigrela, uma fêmea de tigre com quem a mulher partilha o luxo e o conforto de um apartamento, adaptado para tal convivência. Na trama, a mulher ora vela ora revela seus traços animalescos ao mesmo tempo em que acentua atributos humanos na representação do animal. A caracterização dessa relação binária constitui terreno fértil para uma reflexão sobre os limites entre homem e bicho. Sob tal enfoque, faremos uma leitura da narrativa à luz dos conceitos de fantástico formulados por Roger Bozzetto
Abstract:In mimetic discourse by LygiaFagundesTelles, apparently credible events are assumed as imagination, providing a confrontation atmosphere between the strange and the familiar. In her "Tigrela" short story, part of her Mysteries work (1981), the feminine character description is identified by animal aspects that integrate her identity. The story of Romana and her pet, Tigrela -a female tiger with whom the woman shares her wealth and comfort in her apartment, fitted for that convenience. In a spectacular game, the narrator reveals and hides feminine savagery, at the same time he highlights human attributes in his animal representation. The unusual configuration in this binary relation constitutes a fertile soil for a reflection the limits between the human and unhuman, natural and supernatural.
