ÖzetVitiligo, melanositlerin sayı ve işlev kaybı ile karakterize bir pigment hastalığıdır. Etyolojisi bilinmemektedir ancak otoimmun, genetik, biyokimyasal faktörler ve nöral mekanizmalar etyolojide suçlanmaktadır. Melanosit antijenlerini hedef alan sitotoksik T hücrelerinin varlığı ve sitokin seviyelerindeki dengesizliğin melanosit hasarına ve ölümüne neden olduğu düşünülmektedir. Vitiligo tedavisi lezyonların yaygınlığına, lokalizasyonuna, yaşa ve önceki tedavilere verilen yanıta göre değişmektedir. Bu derlemede vitiligo tedavisi geleneksel tedaviler, yeni tedavi seçenekleri, cerrahi tedaviler, deneysel ve diğer tedavi seçenekleri başlıkları altında ele alınmış ve güncel literatür bilgileri ışığında değerlendirilmiştir. (Turk J Dermatol 2010; 4: 33-9 AbstractVitiligo is an pigmentary disorder characterized by loss of functional melanocytes. Altough the aetiology is unknown, autoimmunity, genetic factors, biochemical factors and neural mechanisms were suspected. Cytotoxic T cells targeting melanocyte antigens are present and also an imbalance of the cytokine levels leading to melanocyte damage and death. It is believed that the treatment of vitiligo differs according to the severity of disease, localisation, age and response to previous medications. In this article we reviewed the current literature about vitiligo and classified the treatment options of vitiligo as traditional and current treatment options, surgical treatments, experimental therapies and other treatment options. Sequential displays of several treatment options for vitiligo have been scanned from the literature, and are presented here. A few of the medical modalities have been resigned to history, whereas others have become customary in clinical practice. There has also been a recent surge of interest in the surgical treatment of this disease. Accordingly, this has been appraised and summarized. Special attention has been given to prevalent medical modalities so that they may be effectively utilized by those currently in practice.

Abstract. Melagenine, extracted from human placenta, has been shown to be effective in treating patients with vitiligo, yet the mechanisms of melagenine in inducing the repigmentation of vitiligo patients have not been fully investigated. Recent studies have suggested that melagenine stimulates melanocyte proliferation and melanogenesis. In this study, we utilized the NCCmelb4M5 melanoblast cell line to investigate the effects of melagenine on proliferation and differentiation of immature melanocytes or melanoblasts. NCCmelb4M5 cells were treated with different concentrations of melagenine (50-400 μg/ml), and MTT assay was performed to evaluate the effects of melagenine on proliferation of melanoblasts. RT-PCR and Western blotting were used to determine the expression of c-KIT and tyrosinase (TYR). Our results show that melagenine stimulates proliferation of NCCmelb4M5 cells in a dose-dependent manner with an optimal concentration of 100 μg/ml. Multipolar and highly branched dendritic network, as well as cluster-like growing cell assembly were visible in melagenine-treated NCCmelb4M5 cells. Melagenine induced expression of c-KIT, TYR and MITF. Our results provide insights into the molecular mechanism of the beneficial effect of melagenine in the treatment of vitiligo.
