Neuropathy Following Bariatric Surgery

Abstract: Bariatric surgery is an increasingly common procedure for weight control and is associated with neurologic complications, including disorders of the peripheral nerve. The clinical features of these disorders, steps to take to prevent them, and their prognosis are discussed in this article.

“…Sensory neuropathy subtype occurred less frequently (1%) in intensive nutritional management with frequent follow‐up postoperatively as compared to irregular follow‐up with after care [15]. Patients usually present with painful paresthesia in lower extremities more frequently than the upper extremities (glove‐stocking distribution) [25]; however, similar symptoms have also been reported in the head and trunk [15].…”

“…We identified two high‐quality retrospective controlled cohort studies [15,16], one prospective case series [17], four retrospective case series [18–21], five high‐quality review articles [22–26], and four studies that analyzed the pharmacokinetics of neuropathic analgesics in the PBS scenario [27–30]. No randomized controlled trials, controlled trials on identification of incidence, precise pathology, and management of pain or evidence‐based protocols for therapeutic management were identified.…”

“… 6 Dr. Clark led a discussion about the complications, pathogenesis, prevention, and treatment of neuropathy in bariatric surgery patients. 7 Essentially, the etiology of neuropathy in these patients is not fully understood, although the 2 leading mechanisms are thought to be deficiency of micronutrients and vitamins versus inflammatory changes leading to axonal damage.…”

“…Neurologic complications following bariatric surgery can involve any level of the neuraxis. Particularly important for functioning of the nervous system are the B-group vitamins (vitamin B12, thiamine, niacin, pyridoxine, folate), vitamin E, copper, and vitamin D. Commonly recognized neurologic complications related to bariatric surgery include encephalopathy, optic neuropathy, myelopathy or myeloneuropathy, radiculoplexus neuropathy, polyneuropathy, and mononeuropathy [ 6 , 7 ].…”
