Negative dysphotopsia associated with implantation of the Z9000 intraocular lens

Abstract: Uneventful small-incision cataract surgery was performed in a 70-year-old man and a 62-year-old woman with implantation of a Tecnis Z9000 intraocular lens (IOL) (Pharmacia). No ocular pathology was present in the patients preoperatively. Unsolicited complaints of negative dysphotopsia were made postoperatively, and the symptoms have persisted for more than 1 year in both patients. Symptomatic negative dysphotopsia may be seen in association with the Z9000 IOL.

“…40,41 Nevertheless, there was no difference in disability glare between the groups and no patient was subjectively disturbed by dysphotopsia, as reported elsewhere. 42,43 In summary, the statistically significant reduction in spherical aberration was clinically relevant only in some patients. Thus, selection of patients for aspherical IOL implantation requires careful attention.…”

“…Davison 7 also found these symptoms in eyes with a 1-piece platehaptic acrylic IOL with a nonreflective edge. Narváez et al 11 report negative dysphotopsia symptoms after implantation of an angulated 3-piece silicone IOL with nonreflecting sharp edges. Trattler et al 12 report 3 patients with different IOLs who had typical negative dysphotopsia symptoms in both eyes.…”
