Multifocal Basal‐Cell Carcinomas in a Nevus Sebaceus of Jadassohn

Abstract: A lesion of nevus sebaceus with multifocal basal-cell carcinomas was excised by Mohs' fresh-tissue technique. Histologically, most of the foci of basal-cell carcinomatosis were of the superficial type, but the few more deeply situated foci had adamantinoid features. The malignancies extended well beyond the clinically discernible margins of the nevus sebaceus. Spontaneous healing of the wound of ablation resulted in a satisfactory appearance.

“…Although most of these tumors develop in the third stage of the evolution of NS (during adulthood), malignancies have been reported in children younger than 18 years old. We found that 12% of the BCC (19,22–30), 47% of the SCC (1,31–33), and 21% of the KA (34–36) were reported in children. This percentage may be higher than the incidence of childhood malignancies in NS because authors tend to write reports of unusual cases.…”

“…Prophylactic surgery is classically recommended because the development of various carcinomas on SN has been described 3–6 . BCC is the most frequent malignant tumour occurring on SN, but squamous cell, sebaceous, and apocrine carcinomas have also been reported 13–16 . Locally aggressive or metastatic carcinomas are extremely rare 6,16 and the benefit of systematic prophylactic surgery is currently discussed.…”

“…Furthermore, Rahbari and Mehregan [4]noted the association with proliferating trichilemmal cysts. Basal cell carcinoma (also multifocal) is by far the most commonly reported malignancy [9]. Other malignant neoplasms are squamous cell carcinoma, porocarcinoma, apocrine carcinoma, adnexal carcinoma and sebaceous carcinoma [8].…”
