MRI morphology of the hippocampus in drug-resistant temporal lobe epilepsy: Shape inflation of left hippocampus and correlation of right-sided hippocampal volume and shape with visuospatial function in patients with right-sided TLE

Abstract: We sought to quantify the morphology in vivo of hippocampi in patients with drug resistant temporal lobe epilepsy (TLE) via magnetic resonance imaging (MRI), prior to temporal lobe resection, and the correlation of surface-based shape analysis of morphology and clinical cognitive function.Thirty patients with drug-resistant TLE and twenty healthy controls underwent clinical neuropsychological testing, and brain MRI at Lund University Hospital prior to hippocampal resection. A neuroradiologist categorised radio… Show more

“…We hypothesize that functional compensation after disruption of the left hippocampus could lead to increased activation in the right hippocampus, eventually promoting hippocampal growth. A recent shape analysis study interestingly indicated contralateral inflation of the left hippocampus in RTLE, proposing a similar mechanism of functional compensation 47 …”

“…The different results between the left and right TLE in the corrected analysis may suggest a variance in the intrinsic epileptic networks in patients with unilateral TLE. Another recent study has also confirmed a contralateral shape inflation of the left hippocampus in the right TLE [25]. The connectivity between bilateral temporal lobes has been clearly shown [26,27].…”

“…Speculatively, this phenomenon may be caused by compensatory mechanisms enhancing functions in the left MTL during the course of right MTL epilepsy. In line with this rationale, for instance, increased subsequent memory‐related fMRI activations in the left hippocampus and amygdala (Powell et al., 2007) and an inflation of left hippocampal shape (Yoo et al., 2019) have been reported in right MTL epilepsy patients compared to healthy controls. These findings tentatively suggest that memory‐related functions in the left MTL may surpass normal level during the course of right MTL epilepsy to compensate for the deficient right MTL.…”
