Morphological Analysis in Patients With Sciatica

Abstract: The 3-dimensional DW-SSFP MRI with high spatial and sufficient contrast is an excellent technique to define the nature of sciatica and assists in prognostication and possibly in management.

“…DWIs and ADC maps of spinal nerve roots, ganglions, and nerves with generation of tractography [ 13 , 14 ] were shown to add value for the preoperative assessment of spinal nerve root lesions [ 15 , 16 ]. However, ADC values from the dorsal root ganglion in symptomatic patients were found to be inconsistent, but correlate with recovery from pain and numbness [ 17 ].…”

“…It should be noted that 3D DW PSIF is a sensitive sequence; it is prone to motion and ghosting artifacts. However, MIP reconstructions can enhance apparent SNR and be used to elegantly demonstrate nerves in multiple planes (Figure 4) [3, 6, 20]. Since it is a steady state sequence with both T1 and T2 contrasts, it shows neuropathy and mass lesions as abnormal T2 hyperintensity, with normal nerves appearing isointense to muscles, similar to other anatomic spin echo type sequences.…”

“…This is in part due to the rapid improvements in coil technology and software in the last few years [13]. With improved detection of nerve anatomy and pathology, the value of quantitative functional MR methods as potential biomarkers in neuromuscular disease is also being increasingly recognized [4].…”

“…Normal nerves on 3D DWI-PSIF appear isointense to muscle, while nerve entrapment and resultant neuropathy are demonstrated by an increase in size and signal intensity proximal to the entrapment. 38,39 There have been no reports of quantitative evaluation of the GON signal and contrast characteristics in patients with occipital neuralgia (migraine), to our knowledge. Classically, the SNR has been defined as a measure comparing the level of a desired signal with the level of background noise of the image.…”
