Background: Educational planning and managing critical situations of accidents and disasters are among the most important issues. Triage, which means the classification of patients and injuries based on specific situations and needs, is one of the important tasks of hospitals at times of disasters. In this study, triage knowledge and practice of nurses working in hospitals affiliated to Ilam University of Medical Sciences are evaluated. Materials and Methods: This is a cross-sectional and analytic study conducted in 2017. The statistical population comprised all nurses working in Ilam Medical Sciences hospitals. Using Cochran's formula, 174 people were sampled and 160 of them completed the study questionnaires and returned them. The main tool of collecting data was a researcher-made questionnaire based on Canadian triage scale. Validity of the questionnaire was assessed by 10 members of the Ilam University of Medical Sciences. The reliability of the questionnaire was between 0.83 and 0.89 using Cronbach α coefficient. The obtained data were analyzed using descriptive and inferential statistics in SPSS. Results:The Mean±SD duration of nurses' working in the emergency department was 5.2±3.4 years, of which 46.2% had an experience of using triage. The Mean±SD score of nurses' knowledge and practice of triage were respectively 10.44±2.11 and 9.22±2.14 out of 15. There was no significant relationship between work experience, gender and age with knowledge and practice of triage, but there was a significant relationship between knowledge and practice of nurses about triage with nursing educational degree. Conclusion:The level of nurses' knowledge and practice of triage in hospitals of Ilam University of Medical Sciences is moderate.

RESUMOA classificação de risco em serviços de urgência tem a finalidade de priorizar o atendimento, considerando a gravidade do quadro clínico do paciente. No entanto, há dificuldades em relação ao desenvolvimento dessa atividade pelo enfermeiro. O objetivo do estudo é avaliar a opinião dos enfermeiros sobre a classificação de risco em serviços de urgência. Trata-se de estudo exploratório, quantitativo, com utilização da técnica Delphi. Foram realizadas três rodadas de opinião por meio de questionário disponibilizado em plataforma eletrônica. As alternativas de resposta foram apresentadas de acordo com escala Likert e foi considerado consenso quando o percentual das alternativas de respostas apresentasse frequência igual ou superior a 70%. Para a amostra foram selecionados enfermeiros com experiência profissional e/ou pesquisadores da área. Na primeira rodada participaram 130 enfermeiros, 89 na segunda e 65 na terceira. Os enfermeiros indicaram que a classificação de risco organiza o fluxo de pacientes e diminui o tempo de espera, daqueles em estado grave, por atendimento. Para isso, utilizam o conhecimento clínico, experiência profissional e capacidade de gerenciar conflitos. Os enfermeiros discordaram que a classificação de risco proporciona o acolhimento e privacidade do paciente, assim como sobre a existência de capacitação periódica para o exercício dessa atividade. Concluiu-se que os enfermeiros fortalecem sua prática assistencial na classificação de risco dos pacientes, no entanto, é necessária a elaboração de estratégias para superar as dificuldades estruturais. Background: The clinical decisions of emergency department triage nurses need to be of the highest accuracy. However, studies have found repeatedly that these nurses over- or underestimate the severity of patient health conditions. This has major consequences for patient safety and patient flow management. Workplace distractors such as noise and task interruptions have been pointed to as factors that might explain this inaccuracy. The use of a serious game reproducing the work environment during triage affords the opportunity to explore the impact of these distractors on nurse emergency triage accuracy, in a safe setting. Methods/Design: A pilot study with a factorial design will be carried out to test the acceptability and feasibility of a serious game developed specifically to simulate the triage process in emergency departments and to explore the primary effects of distractors on nurse emergency triage accuracy. Eighty emergency nurses will be randomized into four groups: three groups exposed to different distractors (A: noise, B: task interruptions, C: noise and task interruptions) and one control group. All nurses will have to complete 20 clinical vignettes within two hours. For each vignette, a gold standard assessment will be determined by experts. Pre-tests will be conducted with clinicians and certified emergency nurses to evaluate the appeal of the serious game. Discussion: Study results will inform the design of large-scale investigations and will help identify teaching, training and research areas that require further development.
