Molecular Mechanisms of Colistin Resistance in Klebsiella pneumoniae Causing Bacteremia from India—A First Report

Abstract: Colistin has long been a reserve drug used for the treatment of carbapenem resistant Klebsiella pneumoniae. Carbapenem resistance in K. pneumoniae has been increasing and is as high as 44% in India. Although a reserve agent, with rise in rates of resistance to carbapenems, the usage of colistin has increased over the years leading to slow emergence of resistance. Colistin resistance is mainly mediated by the alteration in the LPS of bacterial outer membrane with the addition of L-Ara4-N and PEtN molecules. The… Show more

“…We also found a high prevalence of the mcr-2 in both pigs and poultry which is surprising as this gene has only been found in a limited number of other studies 11,12,14,15,17,18,22,27 . This is likely because our PCR for mcr is more sensitive in detecting the resistance gene than the conventional methods relying on bacterial isolation as discussed above.…”

“…Colistin is far from an ideal drug, as 50–60% of people taking colistin experience acute kidney injury . Additionally, colistin resistance has emerged, and alarmingly the resistance can be easily spread via horizontal gene transfer of plasmids . The first plasmid associated with colistin resistance was discovered in 2016 in China, but has already spread throughout the world .…”

“…Bacterial resistance has been observed to every antibiotic introduced into the clinic, 1 including drugs-of-last resort such as vancomycin, 2-6 daptomycin, 7,8 and colistin. [9][10][11] The increased rate of resistance, coupled with declining trends in discovery and development of new antibiotics, [12][13][14][15] portends a crisis. Pathogens now exist that are resistant to all or almost all available antibiotics, resulting in an increased number of deaths from bacterial infections.…”

“…The resistance profile for the studied Aeromonas isolates were given in Table 1. Colistin MIC was determined for all the isolates which showed 30% of the isolates were found resistant to 5 colistin. The MIC was identified to be 0.5 µg/ml (susceptible) for the isolate positive for mcr-3 gene.…”
