Sore throat is one of the most frequent reasons for going to the doctor and prescribing irrational antibacterial therapy. The reasons for this are many – from the difficulties of etiological diagnosis and the development of relapses and possible severe complications to the personal attitude of doctors and patients to the choice of treatment. At the same time, the issue of antibiotic resistance, side effects associated with the appointment of antibacterial therapy, is one of the most important problems of modern health care around the world. Difficulties in the process of clinical observation of patients, the peculiarities of the course of infectious and inflammatory diseases of the pharynx are factors requiring constant improvement of methods of providing medical care. Antiseptics play a significant role in the treatment of sore throat, exceeding the frequency of use of antibiotics. So, one of the antiseptics used topically, the effectiveness of which has been proven in clinical studies, is Strepsils, which includes 2,4-dichlorobenzyl alcohol and amylmetacresol. The results of clinical studies of the drug indicate its positive effect in patients with sore throat. All this things allow us to recommend that this complex antiseptic be more often included in the treatment of infectious and inflammatory diseases of the oral cavity and pharynx.

Metallurgy is a major economic branch in Russia with more than 4000 enterprises operating in it and seventy percent of them being city-forming ones. This study focuses on cytological assessment of the oral mucosa and secretion from the middle meatus mucosa in workers employed in metallurgy. The aim of this study was to investigate cytological laboratory indicators in workers employed in metallurgy under exposure to adverse occupational factors. A clinical and diagnostic examination of workers employed at a metallurgical plant in Bashkortostan was performed in 2019–2020; it involved cytological studies of the oral mucosa (buccal epithelium) and the middle meatus mucosa (rhinocytogram). In this study, we applied the Index of cytogenetic disorders accumulation (Iac) that allows for cellular kinetics indicators. The overall hygienic assessment of working conditions for workers employed at the analyzed metallurgic plant corresponds to the hazard category 3.2–3.3 in accordance with the criteria outlined in the Guide R (harmful, class 2 or 3). The research results revealed cytogenetic disorders of buccal epithelial cells in the workers who had contacts with adverse occupational factors. Low likelihood of cytogenetic disorders was established for 66.67 % of the workers; moderate, 9.2 %; high, 23.81 %. We assessed rhinocytograms of the workers exposed to adverse occupational factors and revealed some signs of allergic inflammation characterized with high eosinophil count. The research results confirm high significance of diagnostic procedures for developing an algorithm for screening examinations of working population as well as indicators of health disorders under exposure to adverse occupational factors (noise, heating microclimate, industrial dust, gaseous chemicals).

Application of a herbal preparation in the therapy of acute tonzillopharyngitis in children

Introduction. Tonsillopharyngitis often accompanies the course of acute respiratory infections in children (up to 12-15%). The use of systemic antibacterial drugs for viral etiology of the disease has not been shown, therefore, the interest of researchers in the use of herbal products is increasing. In numerous clinical trials conducted around the world, the liquid extract of the roots of Pelargonium sidoides EPs® 7630 has been shown to be effective and safe in the treatment of acute respiratory infections, acute tonsillopharyngitis, rhinosinusitis, acute bronchitis in adults and children.Objective of the study: assessment of the efficacy and safety of the drug EPs® 7630 (Umckalor) in acute tonsillopharyngitis of non-streptococcal etiology in children 2-6 years old on an outpatient basis.Materials and methods. A prospective analysis of cases of treatment with Umckalor for acute tonsillopharyngitis of non-streptococcal etiology against the background of acute respiratory infection in children 2-6 years old (n = 23, group 1) was carried out.The comparison group (2) consisted of children receiving standard therapy (n = 22). Systemic antibiotics and antiviral drugs were not used in all patients. The study was conducted over 6 months (2019-2020). The research results were processed using the Statistica 7.0 software (StatSoft, USA).Results and discussion. Therapy with Umckalor in children with acute tonsillopharyngitis contributed to the elimination of the main clinical symptoms of the disease on the 4th day of treatment (efficiency was 69.9%, p < 0.05). The most persistent symptom was hyperemia of the tonsils and oropharynx. The period of convalescence occurred 2 days earlier than in the children of group 2 (p < 0.05). Umckalor was well tolerated, no side effects of the drug were identified.Conclusions. The drug Umckalor can be recommended for monotherapy for 7 days in patients with acute non-streptococcal tonsillopharyngitis on an outpatient basis with mild and moderate disease.
