MitraClip patient selection: inclusion and exclusion criteria for optimal outcomes

Abstract: Edge-to-edge repair with the percutaneous MitraClip technology has changed the landscape for patients with symptomatic, severe degenerative mitral valve regurgitation who are at prohibitive surgical risk. While the results of randomized controlled trials of MitraClip therapy in patients with functional mitral valve regurgitation are still pending, single center experiences as well as registry data generally support the realworld application of the MitraClip therapy. In the majority of individuals treated with … Show more

“…Compared with the restrictive inclusion criteria of the EVEREST trial (20, 21), nowadays more patients with challenging anatomy are referred for percutaneous edge-to-edge repair (22). Except for a mitral valve orifice area (MVOA) <4 cm 2 in COAPT, no specific anatomic exclusion criteria are applied in the most recent randomized trials, MITRA-FR (9) and COAPT (5).…”

“…Indeed, comparisons of three-dimensional (3D) transesophageal echocardiography and cardiac CT have shown that both imaging modalities provide good detailing of mitral leaflet morphology ( 58 ). In addition, calcification and clefts of the leaflets are important to note as these can preclude adequate transcatheter edge to edge repair (TEER) ( 59 ).…”

“…Recommended criteria for selecting patients who are suitable to TMVR are reported elsewhere. 18 All patients received PFTs during primary investigation, which is a mandatory part of standard operating procedures (SOPs) at our institution. Wholebody plethysmography (MasterLab, Viasys) was performed before and after bronchodilation according to current guidelines and recommendations.…”
