Introduction: Recently, the identification of minimal residual disease (MRD) that persists after chemotherapy has emerged as the most powerful tool in determining the prognosis of patients with acute lymphoblastic leukemia (ALL). Multiple methods to detect MRD exist, each with its own benefits and drawback. Multiparameter flow cytometry and quantitative polymerase chain reaction are the most commonly used methods of MRD detection in clinical practice. Objective: to evaluate the impact of minimal residual disease detection by Next Generation Flow Cytometry on Outcome of Egyptian Patients with Acute Lymphoblastic Leukemia. Patients &Methods: The study conducted on 93 patients with recently diagnosed acute lymphoblastic leukemia. MRD detection was evaluated during follow up of patient (at End of induction EOI and End of consolidation EOC by next generation flow cytometry. Results: Out of 93 patients, 28 (30%) had positive MRD at EOI. Age, BCR-ABL, risk assessment, and relapse had a substantial impact on MRD at EOI (P <0.005). Fourteen patients (17.9%) at EOC were MRD positive; age, hemoglobin, blast count at diagnosis, BCR-ABL, risk stratification, relapse and overall survival showed significant association. Conclusion: Positive MRD was a major risk factor for predicting poor survival and relapse at both EOI and EOC by cox regression analysis.

Minimal residual disease (MRD) assessment in patients with acute lymphoblastic leukemia (ALL) became an integral part of total evaluation of chemotherapy efficacy. No doubt, that it is necessary to evaluate MRD at remission induction and before allogenic stem cell transplantation, but MRD prognostic role at post-induction treatment is on-study.Aim. To evaluate a prognostic significance of MRD at post-induction treatment in pediatric patients with ALL.Materials and methods. From October 2010 to September 2022, there were 142 pediatric patients with primary diagnosed B-precursor ALL enrolled the study. All the patients were treated by ALL–IC BFM 2009 protocol.Results. 6-year overall survival (OS) for patients with MRD-positive status at post-induction treatment (78th day of therapy) was 90.8 ± 4.0 %, with MRD-negative – 91,1 ± 3.9 % (р = 0,8). Relapse-free survival (RFS) for MRD-positive – 67.4 ± 11.6 %, MRD-negative – 88.9 ± 4.3 % (р = 0,03). Event-free survival for MRD-positive – 67.4 ± 11.6 %, MRD-negative – 87,5 ± 4.5 % (р = 0,06).Conclusion. MRD level at induction remission treatment is an important risk-stratified factor in pediatric patients with ALL, but prognostic significance of MRD at post-induction therapy is under investigation. Presented our data showed increased relapse incidence on 21.5 % in patients with MRDpositive status at 78th day of therapy. OS was the same and not depended on post-induction MRD-level.

Background: Chromosome-1 abnormalities (C1As) are common genetic aberrations in hematological malignancies. We sought to evaluate significance of these abnormalities with reference to clinical characteristics and survival outcome in a pediatric B-Lymphoblastic Leukemia (B-ALL) cohort. Methods: This is a retrospective study conducted in cytogenetic section of Indus Hospital and Health Network. Data was retrieved from October 2020 to July 2022 for childhood B-ALL cases exhibiting C1As. Chromosome analysis was performed on Cytovision MB8 using G-banded metaphases derived from unstimulated bone marrow culture. Results were recorded according to the International System for Human Cytogenetic Nomenclature (ISCN-2020). Data analyzed using SPSS, version 24.0. Results: C1As were observed in 60/450 (13.3%) cases of B-ALL. Among C1As, 29 (48%) cases had t(1;19). There were 13 (45%) balanced and 16 (55%) unbalanced translocations. The aberrations without t(1;19) were seen in 31 (52%) cases including 1q duplication with hyperdiploidy in 14 (45%) cases. The median age for C1As with and without t(1;19) was eight years and six years while the median leukocyte count was 32 x 109/L vs. 17 x 109/L. Event-free survival (EFS) for cases with and without t(1;19) was 69% and 74.2% respectively. Conclusion: Despite the fact that the t(1;19) positive group had a higher median age, a higher white cell count and more CNS positives, the difference in EFS is statistically insignificant when compared to the t(1;19) negative cases.
