Metronidazole-induced encephalopathy

Abstract: Metronidazole is a widely used antibacterial and antiprotozoal agent for a number of conditions. Whilst its more common gastrointestinal side effects are well known, neurotoxicity remains under-recognised. Both central and peripheral neurological side effects have been described. This report describes a case of radiologically confirmed metronidazole-induced cerebellar ataxia in a cirrhotic patient with a review of the literature. Awareness of this side effect is essential for prompt recognition as early drug w… Show more

“…In multiple sclerosis, hyperintense signals on T2WI may be present in the periventricular, cortical, juxtacortical, infratentorial, and brain stem, with active lesions showcasing true restriction diffusion and contrast enhancement [ 20 ]. If the splenium shows T2-weighted hyperintensities, other conditions such as Marchiafava-Bignami, encephalitis, epilepsy, extrapontine osmotic demyelination, lupus erythematosus, renal failure, and cobalamin deficiency must be ruled out [ 21 ]. Heat stroke affects the thalami and external capsules, whereas cerebellar involvement is rare [ 22 ].…”

“…1 It is crucial to optimize the dosage and duration of metronidazole use, as a prolonged period of administering more than 2 g/day is associated with peripheral neuropathy and cerebellar dysfunction. 2,3 Gut translocation of bacteria with clinically significant septic complications is common in bowel obstruction. In such cases, broad-spectrum antibiotics, including metronidazole, are frequently employed as an adjunct to surgery.…”
