Female genital tract (FGT) inflammation increases HIV infection susceptibility. Non-optimal cervicovaginal microbiota, characterized by depletion of Lactobacillus species and increased bacterial diversity, is associated with increased FGT cytokine production. Lactobacillus species may protect against HIV partly by reducing FGT inflammation. We isolated 80 lactobacilli from South African women with non-optimal (Nugent 4-10; n = 18) and optimal microbiota (Nugent 0-3; n = 14). Cytokine production by vaginal epithelial cells in response to lactobacilli in the presence and absence of Gardnerella vaginalis was measured using Luminex. Adhesion to vaginal epithelial cells, pH, D/L-lactate production and lactate dehydrogenase relative abundance were assessed. Lactobacilli from women with non-optimal produced less lactic acid and induced greater inflammatory cytokine production than those from women with optimal microbiota, with IL-6, IL-8, IL-1α, IL-1β and MIP-1α/β production significantly elevated. Overall, lactobacilli suppressed IL-6 (adjusted p < 0.001) and IL-8 (adjusted p = 0.0170) responses to G. vaginalis. Cytokine responses to the lactobacilli were inversely associated with lactobacilli adhesion to epithelial cells and D-lactate dehydrogenase relative abundance. Thus, while cervicovaginal lactobacilli reduced the production of the majority of inflammatory cytokines in response to G. vaginalis, isolates from women with non-optimal microbiota were more inflammatory and produced less lactic acid than isolates from women with optimal microbiota.

Kratak sadržajUvod. Bakterijska vaginoza predstavlja poremećaj u sastavu vaginalne flore u smislu smanjenja broja laktobacila, a predominacije anaerobnih bakterija Gardnerella vaginalis, Atopobium vaginae i Mobiluncus spp. Najčešći simptom udružen sa bakterijskom vaginozom je rijedak, homogen, mliječno bijeli ili sivkast vaginalni sekret, neprijatnog mirisa. Cilj rada je određivanje učestalosti i simptomatologije izmijenjene vaginalne flore kod žena u reproduktivnom periodu iz područja opštine Foča, Republika Srpska. Metode.Ispitivanje je obuhvatilo 100 žena u reproduktivnom periodu, pri čemu trudnice nisu uključene u ovu studiju. Na direktnom preparatu obojenom po Gram-u, koristeći sistem bodovanja po Nugent-u, dijagnostikovana je izmijenjena vaginalna flora. Na osnovu anamnestičkih podataka utvrđivali smo da li se pojedini simptomi češće javljaju u grupi ispitanica sa izmijenjenom vaginalnom florom (bakterijska vaginoza i intermedijerna flora) u odnosu na žene sa normalnom vaginalnom florom.Rezultati. Koristeći sistem bodovanja po Nugent-u bakterijska vaginoza je dijagnostikovana kod 22% ispitanica, intermedijerna flora je nađena kod 18% ispitanica, dok je njih 60% imalo normalnu vaginalnu floru. Kod ispitanica sa dijagnostikovanom bakterijskom vaginozom najčešći simptom je bio pojačan sekret. Obradom podataka utvrđeno je da nema statistički značajne razlike u učestalosti ispitivanih simptoma (pojačan sekret, peckanje, svrab) kod ispitanica sa izmijenjenom vaginalnom florom u odnosu na ispitanice sa normalnom vaginalnom florom. Asimptomatska bakterijska vaginoza je dijagnostikovana kod 4 (18,2%) ispitanice.Zaključak. Izmijenjena vaginalna flora, u vidu bakterijske vaginoze ili intermedijerne flore dijagnostikovana je kod 40% ispitanica. Među ispitanicama sa dijagnostikovanom bakterijskom vaginozom najčešći simptom je bio pojačan sekret ali nisu uočene značajne razlike u učestalosti simptomatoma kod žena sa izmijenjenom vaginalnom florom u odnosu na žene sa normalnom vaginalnom florom.Ključne riječi: bakterijska vaginoza, vaginalni sekret, vaginalna mikroflora

This article reviews the findings of the Phase II clinical trial conducted in 80 women suffering from recurrent episodes of Vaginosis whose vaginal pH was above 5.0. Intravaginal intake every night for seven days of easily dispersible capsules containing either the Polyherbal wide spectrum action microbicide BASANT, or 3 × 10 9 bacilli of 3 selected meritorious Probiotics cured Vaginosis in 65%-70% of women. However, the intake of the Combination of the two was remarkably effective in curing 95% (19 out of 20) women, restoring fully healthy vagina.
