Meta-analysis of posterior versus medial approach for popliteal artery aneurysm repair

Abstract: High-level comparative data comparing posterior and medial repair for popliteal artery aneurysms are not available. Within the parameters of this review, however, superiority of the posterior approach for primary and secondary patency, aneurysm exclusion, and need for reoperation was noted. High-level evidence from randomized clinical trials is required to define the relative benefits of the posterior approach over the medial approach in selected patients.

“…A meta-analysis of seven comparative, nonrandomised studies including 338 patients undergoing posterior and 1 089 undergoing medial open bypass included a majority of elective repairs. The posterior approach was superior in terms of primary and secondary patency, aneurysm exclusion, and need for re-operation, 177 although it was more often done for short lesions (Fig. 9).…”

“…Symptomatic PAAs of any size are treated by either surgical ligation combined with autologous vein bypass or by endoprosthesis. Surgical treatment may be the preferred emergency intervention.…”

“…[1][2][3] Surgery is recommended for popliteal aneurysms with diameters greater than two centimeters or those that are symptomatic. 4 Treatment may be with open surgery, which is the most widely-used approach, or endovascular surgery, using an endoprosthesis. 5 Traditional open surgical treatment consists of proximal and distal ligature of the popliteal artery aneurysm to exclude the segment, with a venous bypass to divert flow around the aneurysm (although a prosthetic graft is also an option).…”
