Hintergrund: Angst und Schmerz bestimmen das Atmungsverhalten und den Muskeltonus der werdenden Mutter unter der Geburt. Eine adä-quate Atmung in der Austreibungs-und Pressperiode wirkt sich schmerzlindernd aus und för-dert die Entspannung -auch der glatten Muskulatur. Mutter und Kind sind bis zur Geburt eine funktionelle Einheit. Unter bestimmten Bedingungen ist das Atmungsverhalten der Mutter an den aktuellen Blutgasen (pCO 2 und O 2 ) und ihr Muskeltonus am BE des Fetus im NV-Blut direkt ablesbar. In dieser Studie wird der Versuch unternommen, diese Zusammenhänge darzustellen, zu quantifizieren und in ihrer forensisch-klinischen Bedeutung zu evaluieren. Methodik: Bei 101 Patientinnen unterschiedlicher Parität wurden genau zum Zeitpunkt der Geburt arterielle Blutgasanalysen (Art. radialis) gemacht und der SB-Status im NA-und NV-Blut bestimmt. Unabhängig davon wurde bei 453 Feten von Müttern unterschiedlicher Parität das direkt abgeleitete CTG während der letzten 30 Minuten ante partum elektronisch (CTG-Score) analysiert. 60 dieser Feten wiesen eine minimale CTG-Pathologie auf (Score = 0). Die blutgasanalytischen Werte im NV-Blut dieser Feten dienten als "Spiegel" der mütterlichen Einflüsse. In einer weiteren, separaten Stichprobe von 4853 Feten, die unter "Standardbedingungen" in völlig lebensfrischem Zustand (Apgar 1 min: 9/10) spontan und termingerecht zur Welt gekommen waren, wurden die blutgasanalytischen Daten in Abhängigkeit von der Parität analysiert. Die durch die mütterlichen Einflüsse (Atmung und Muskelarbeit) verursachte, physiologische Variabilität der pH-Werte im NV-und NA-Blut wurde aus den Absolutkonzentrationen der H + -Ionen (cH + ), also über die Numeri, ermittelt (nmol/l) und dann in pH-Werte zurückgerechnet. Therefore maternal breathing habits can be deduced from the fetal acid-base status (ABS) of blood from the umbilical vein when the fetus stays unimpaired in utero. This study investigated and quantified this unique maternofetal relationship. Methods: Three different samples from fetuses were studied: 1. At the exact time of delivery, the ABS in arterial and venous umbilical blood (UA and UV) was determined in 101 patients and an analysis of maternal arterial blood gases (radial artery) was carried out. 2. Direct monitoring of the fetal heart rate (FHR) was performed in 453 fetuses of mothers with various parities. A new FHR score, which has been developed in this institution, was used for electronic FHR analysis. A minimal FHR pathology (score = 0) was encountered in 60 of these 453 fetuses during the last 30 min of labor.
