Mephedrone (4-methylmethcathinone) is a novel psychoactive substance popular among drug users because it displays similar effects to MDMA (3,4-methylenedioxymethamphetamine, ecstasy). Mephedrone consumption has been associated with undesirable effects and fatal intoxications. At present, there is no research available on its pharmacological effects in humans under controlled and experimental administration. This study aims to evaluate the clinical pharmacology of mephedrone and its relative abuse liability compared with MDMA. Twelve male volunteers participated in a randomized, double-blind, crossover, and placebo-controlled trial. The single oral dose conditions were: mephedrone 200 mg, MDMA 100 mg, and placebo. Outcome variables included physiological, subjective, and psychomotor effects, and pharmacokinetic parameters. The protocol was registered in ClinicalTrials.gov (NCT02232789). Mephedrone produced a significant increase in systolic and diastolic blood pressure, heart rate, and pupillary diameter. It elicited stimulant-like effects, euphoria, and well-being, and induced mild changes in perceptions with similar ratings to those observed after MDMA administration although effects peaked earlier and were shorter in duration. Maximal plasma concentration values for mephedrone and MDMA peaked at 1.25 h and 2.00 h, respectively. The elimination half-life for mephedrone was 2.15 h and 7.89 h for MDMA. In a similar manner to MDMA, mephedrone exhibits high abuse liability. Its earlier onset and shorter duration of effects, probably related to its short elimination half-life, could explain a more compulsive pattern of use as described by the users.

Mephedrone is a drug of abuse marketed as ‘bath salts". There are discrepancies concerning its long-term effects. We have investigated the neurotoxicity of mephedrone in mice following different exposition schedules. Schedule 1: four doses of 50 mg/kg. Schedule 2: four doses of 25 mg/kg. Schedule 3: three daily doses of 25 mg/kg, for two consecutive days. All schedules induced, in some animals, an aggressive behavior and hyperthermia as well as a decrease in weight gain. Mephedrone (schedule 1) induced dopaminergic and serotoninergic neurotoxicity that persisted 7 days after exposition. At a lower dose (schedule 2) only a transient dopaminergic injury was found. In the weekend consumption pattern (schedule 3), mephedrone induced dopamine and serotonin transporter loss that was accompanied by a decrease in tyrosine hydroxylase and tryptophan hydroxylase 2 expression one week after exposition. Also, mephedrone induced a depressive-like behavior, as well as a reduction in striatal D2 density, suggesting higher susceptibility to addictive drugs. In cultured cortical neurons, mephedrone induced a concentration-dependent cytotoxic effect. Using repeated doses for 2 days in an elevated ambient temperature we evidenced a loss of frontal cortex dopaminergic and hippocampal serotoninergic neuronal markers that suggest injuries at nerve endings.

StreszczenieKokaina i pochodne amfetamin są zdaniem większości autorów najczęstszymi środkami psychoaktywnymi, których stosowanie wiąże się z rozwojem ostrego rozwarstwienia aorty (acute aortic dissection -AAD). W przedstawionym przypadku AAD mogło mieć związek z rekreacyjnym używaniem mefedronu. Choć środek ten uważany był do niedawna za stosunkowo bezpieczny, w ostatnich latach doniesiono o kilku przypadkach śmiertelnych spowodowanych jego stosowaniem. W pracy przedstawiono przypadek 29-letniego mężczyzny, który został przyjęty do ośrodka toksykologii z powodu przedawkowania mefedronu. W badaniu przedmiotowym dominowały silne bóle kończyn dolnych oraz okolicy moszny. W badaniach dodatkowych stwierdzono rozwarstwienie aorty wstępującej, łuku oraz części zstępującej, a także liczne skrzepliny w tętnicach jamy brzusznej i obu kończyn dolnych. Pomimo podjęcia natychmiastowego leczenia operacyjnego doszło do rozwinięcia się uszkodzeń wielonarządowych, które doprowadziły w rezultacie do zgonu chorego. Słowa kluczowe: mefedron, ostre zatrucie, rozwarstwienie aorty, zatorowość. AbstractAcute aortic dissection (AAD) is mainly connected with the use of cocaine and amphetamines. We describe the AAD which can be related to the recreational use of mephedrone. Mephedrone was regarded as a relatively safe psychoactive substance; however, there were some medical papers recently about death caused by the use of this substance. We describe a 29-year-old man who was admitted to the toxicology center after overdose of mephedrone. He complained of scrotal and lower limb pain. Dissection of ascending arch, descending aorta and emboli of abdominal and lower limb arteries was found in imaging studies. Even though the surgical procedures were performed immediately the patient died in the course of multiorgan failure. Key words: mephedrone, acute intoxication, aortic dissection, embolism. Ostre rozwarstwienie aorty w przebiegu zatrucia mefedronem -opis przypadku WstępMefedron (4-metylometkatinon) jest syntetyczną pochodną katinonu, alkaloidu obecnego w liściach i łodydze krzewu Catha edulis. Roślina występuje naturalnie w Afryce i na Bliskim Wschodzie. W ostatnich latach mefedron stał się popularnym narkotykiem oraz składnikiem wielu różnych "dopalaczy" stosowanych przez młodzież w celach rekreacyjnych [1][2][3]. Na rynku europejskim środek ten obecny jest w postaci proszku, rzadziej tabletek czy kapsułek. Zażywany jest donosowo lub doustnie, bywa także podawany doodbytniczo oraz w postaci wstrzyknięć dożylnych i domięśniowych.
