A case series of haematoperitoneum caused by ovarian bleeding after transvaginal oocyte retrieval (TVOR) is presented and all published cases summarized. In a retrospective case series, four patients with ovarian bleeding after TVOR were included. In addition, a pooled analysis of all published cases (n = 32) who underwent surgical intervention for severe haematoperitoneum caused by ovarian bleeding after TVOR was carried out. Main outcome measures were incidence, risk factors, course and intraoperative findings. In the pooled analysis, the incidence was 0.08%. The first sign of haematoperitoneum was evident in 33.3% within the first postoperative hour, and, cumulatively, in 93.3% within 24 h. The median time between TVOR and surgical intervention was 10 h. In four patients, the ovary could not be preserved, which was associated with a longer time interval between TVOR and the onset of symptoms (median 18 h versus 2.5 h; P = 0.004) as well as between TVOR and surgical intervention (median 21.5 h versus 8.5 h; 0.004). In conclusion, severe haematoperitoneum occurs in 0.08% after TVOR. Late-onset bleeding is common. A longer time interval between TVOR and surgical intervention might put a patient at risk of ovariectomy.

Background: Complications associated with oocyte retrieval include ovarian hyperstimulation syndrome, ovarian torsion, infection, bleeding, and even acute urinary retention. Case: A 29 year old female presented to the emergency department for dizziness and syncope following oocyte retrieval and was admitted for ovarian hyperstimulation syndrome. Her urinary catheter was obstructed by blood clots and relieved with bladder irrigated. The catheter was removed after twenty-four hours, and she was able to void spontaneously. Four days after discharge, she returned for urinary retention. She underwent cystoscopy and 10cc of blood clots was evacuated. The bladder appeared normal without intravesicular bleeding, and she was discharged home. Twelve hours later, the patient returned for urinary retention. Another cystoscopy was performed and 150cc of blood clots was evacuation. Her symptoms resolved. Conclusion: Hematuria and urinary retention are rare but serious complications of oocyte retrieval. Management options include a urinary catheter, bladder irrigation, urologic imaging, and cystoscopy

Evaluation of complications developing during and after transvaginal ultrasound — guided oocyte retrieval

Objectives: To investigate the complications of the oocyte retrieval procedure currently used in in vitro fertilisation. Material and methods:We examined 1.031 patients who underwent oocyte retrieval in the IVF unit of our hospital for complications developed during and after the procedure. Results:No complications developed related to sedation or general anaesthesia. Vaginal bleeding was observed in 3.1% of the patients. There was no intra-abdominal bleeding or pelvic organ injuries requiring surgery. Two patients developed pelvic abscesses. Ovarian hyperstimulation syndrome (OHSS) occurred in 1.45% of the patients. Almost all of the patients tolerated the oocyte retrieval process well. After the procedure, only 2% of the patients described their pain as severe, and 0.4% as the worst pain they had ever experienced. Conclusions:The most common complication during oocyte retrieval is vaginal bleeding, which is largely controlled by buffer application. In conclusion, the oocyte retrieval process can be considered a safe procedure.
