Objective: Compared to the cutaneous melanoma, noncutaneous melanomas are relatively rare and have a distinct pattern of behavior. We performed this study to define the clinical characteristics and outcomes of patients with noncutaneous melanomas and emphasize how they differ from cutaneous melanomas. Methods: 216 patients who were diagnosed with noncutaneous melanoma were assessed and their medical records between 2000 and 2010 were retrieved from the cancer registry. 475 patients with cutaneous melanoma were used for comparison. Results: Of the 216 noncutaneous melanoma patients, 83 had mucosal melanoma, 101 had ocular melanoma and 32 had unknown primaries. For mucosal melanoma, the head and neck was the most common anatomic site (53%), followed by the gastrointestinal tract (37%), female genital (6%) and urinary tract (4%). The majority of the ocular melanomas (94%) originated in the uvea. 32 patients demonstrated a primary unknown disease with regional metastasis as the dominant behavior (72%). Age was found to be statistically significantly different among melanoma patients (p < 0.001). Younger patients had more cutaneous and unknown primary melanomas, whereas older patients had more ocular and mucosal melanoma. In subset analyses, we found significant differences between cutaneous and ocular (p = 0.038) or mucosal (p < 0.001) melanomas. The ratios of metastasis on admission were higher in patients with mucosal (27.7%) and unknown primary (28.1%) melanomas, and lower for ocular (3%) melanomas (p < 0.001). Patients with cutaneous melanoma had an intermediate (12%) ratio. Patients with ocular melanoma had better outcome than patients with other melanoma sites (p < 0.05). While overall survival for cutaneous melanoma was significantly negatively correlated with male gender (p < 0.001), advanced stages (p < 0.001) and old age (p = 0.005), stage IV disease was the only independent prognostic factor in patients with ocular melanoma (p = 0.006). No prognostic factor for outcome was found for either mucosal or unknown primary melanomas. Conclusion: The clinical presentations and prognoses of different primary sites of melanoma are distinctive. Therefore, approaches to these melanomas should be different.

Résumé -Le mélanome est une tumeur qui se développe à partir des cellules pigmentaires dérivant des crêtes neurales. Les mélanomes buccaux constituent 0,4 à 8 % de l'ensemble des tumeurs mélaniques. Ils atteignent plus l'homme entre 50 et 60 ans, et se développent essentiellement sur la muqueuse palatine, la gencive, les lèvres. L'étiopathogénie demeure inconnue; il existerait cependant une prédisposition génétique mais aucun précurseur bien défini n'a été isolé. Les microtraumatismes répétés sur un naevus bénin pré-existant ont également été incriminés. L'évolution du mélanome se fait en 2 phases : d'abord horizontalement, puis verticalement. La très haute malignité, l'agressivité et le sombre pronostic de cette tumeur en font une entité dont le diagnostic précoce permet d'améliorer de façon importante le taux de survie des patients. Nous présentons 3 cas de mélanomes dont le diagnostic tardif a rendu la prise en charge difficile et le pronostic sombre.Abstract -Oral melanoma: 3 cases report. The melanoma is a tumor which develops from the pigmentary cells diverting neural crests. The oral melanomas constitute 0.4 to 8% of all the melanic tumors. They affect more the men between 50 and 60 years, essentially at the palatine mucous membrane, the gum, the lips. The etiopathogeny is unknown; there would be however a genetic predisposition but no well defined precursor was isolated. The repeated microtraumas on a pre-existing mild naevus were also incriminated. The evolution is made in 2 phases: at first horizontally then vertically. The very high wickedness, the aggressiveness and the somber forecast of these tumors make it entities in which the early diagnosis allows to improve the rate of survival of the patients. We present through this work 3 cases of oral melanomas; the late diagnosis maked the care difficult and the prognosis dark.

Orale maligne Melanome (OMM) sind selten; sie machen weniger als 0,5% aller malignen Erkrankungen im Mundraum aus. Die häufigsten Lokalisationen sind der Gaumen und die maxilläre Gingiva. Die histologische Untersuchung ist von entscheidender Bedeutung für die Diagnosestellung bei verdächtigen pigmentierten Läsionen in der Mundhöhle, vor allem, wenn eine präzise klinische Diagnose nicht möglich ist. Wir stellen einen Fall von OMM vor, der 2 Jahre zuvor in einem anderen Zentrum als ethniespezifische Pigmentierung fehldiagnostiziert worden war. Die klinische Untersuchung ergab multiple Makulae und Noduli auf dem harten und weichen Gaumen, der oberen Alveolarmukosa und Gingiva. Diese Läsionen waren schmerzlos und variierten farblich von dunkelblau bis schwarz. Die histologische Analyse zeigte Lagen und Nester atypischer Melanozyten verschiedener Formen (plasmazytoid, epithelioid, rund) im oberflächlichen Korium mit Ausbreitung in tiefere Gewebeschichten. Manche Tumorzellen enthielten variable Mengen an Melanin. Eingesprosste Blutgefäße oder Nervenfasern waren nicht zu sehen. Die immunhistochemische Analyse ergab, dass die neoplastischen Zellen positiv für HMB-45, Melan-A und S-100 und negativ für AE1/AE3 waren, was die Melanomdiagnose bestätigte. Der Ki-67-Labeling-Index lag bei rund 25%. Die Patientin lehnte jegliche Behandlung ab und verstarb 11 Monate später. Übersetzung aus Dermatopathology 2016;3:1-7 (DOI:10.1159/000444049)

Abstract: The standard therapy for melanoma continues to be surgical resection, possibly associated with adjuvant radiation. Biochemotherapy should be considered for bulky metastatic disease. In the future, definitive radiation regimes, molecular staging and targeted therapy may play a major role.
