Malignant Hidroacanthoma simplex

Abstract: A 73-year-old woman with a malignant hidroacanthoma simplex on her right lower limb is reported. The lesion was immunohistochemically studied. Carcinoembryonic antigen and epithelial membrane antigen were found in the luminal border of ductal structures within the aggregations of neoplastic cells. We review the literature on this subject.

“…Examples of malignant variants of HAS have been reported under different names, mainly as malignant HAS, in situ porocarcinoma, eccrine porocarcinoma, and HAS with invasive growth. Pagetoid Bowen's disease probably presents the greatest potential for histopathologic differential diagnosis with our case of malignant HAS 3 . However, Bowen's disease usually shows evidence of keratinization of neoplastic cells in the form of dyskeratoses, and ductal differentiation is absent 6 …”

“…HIDROACANTHOMA SIMPLEX (HAS) is a rare benign intraepidermal eccrine neoplasm clinically resembling seborrheic keratosis or Bowen's disease 1,2 . Malignant transformations of HAS are reported in the literature as a various name with some metastatic potential 3 …”

“…As such, it has been called atypical poroma [1] or porocarcinoma in situ [29]; the overtly cytological ‘malignant' changes of this case could represent a malignant progression of EP towards aggressive EPC and could explain the presence of residual EP in a variable percentage of EPCs [1,2]. These findings are mostly superimposable in the so-called malignant hidroacanthoma simplex that is considered an in situ variant of porocarcinoma [30]. Also, the discrepancy between the relatively common EP and the rare EPC can be explained considering that the diagnosis of EPC is often missed in favor of other more common skin neoplasms, i.e.…”

“…CO 2 laser treatment of her recurred lesion was a simple solution which required less healing time than the topical 5-FU ointment application. Although a few cases of malignant hidroacanthoma simplex have been reported (7)(8)(9), this case showed no malignant transformation and responded well to the conservative treatment.…”
