Zusammenfassung Nach einem Schlaganfall wurde ein 51 Jahre alter Mann in eine Rehaklinik verlegt. Gemäß klinischen Angaben soll sich dort eine Sepsis entwickelt haben. Aufgrund eines plötzlichen Bewusstseinsverlustes wurde der Patient umgehend in die nächstgelegene Klinik verbracht, wo er trotz umgehender Behandlung verstarb. Da die Ehefrau Vorwürfe gegen das Personal der Rehaklinik erhob, wurde eine rechtsmedizinische Obduktion in Auftrag gegeben. Im Rahmen der Obduktion fielen große Mengen flüssigen Blutes im Magen sowie im oberen Gastrointestinaltrakt auf, sodass bereits autoptisch ein inneres Verbluten als Todesursache definiert wurde. Als Blutungsquelle kam ein makromorphologisch auffälliges, eröffnetes Gefäß im Magenfundus in Betracht. Die sichere Diagnose eines tödlichen Dieulafoy-Ulkus konnte jedoch erst nach histologischer Begutachtung erfolgen. Definierend für diesen Befund ist eine kaliberpersistente Arterie in der Submukosa ohne pathologische Veränderungen. Bei sicherer Diagnose kann dieser Befund einen plötzlichen Tod aus natürlicher Ursache darstellen. Da das Ulkus im gesamten gastrointestinalen Trakt vorkommen kann, ist bei relevantem Blutverlust und unklarer Blutungsquelle neben der genauen Inspektion eine histologische Aufarbeitung möglicher Blutungsquellen unerlässlich. Damit die Kriterien eines Dieulafoy-Ulkus beurteilt werden können, sollte neben einer HE(Hämatoxylin-Eosin)-Färbung mindestens eine EvG(Elastica-van-Gieson)-Färbung durchgeführt werden. Nur dann können alternative Entstehungsmöglichkeiten ausgeschlossen werden, welche aus forensischer Sicht möglicherweise hätten verhindert werden können.

This paper reviews the gross morphology, histologic classification, location, and modes of spread of gastric cancer. On the basis of gross appearance, we have classified gastric carcinoma into 4 types: (a) superficial spreading (6% of cases), (b) polypoid (11% of cases), (c) ulcerated (45% of cases), and (d) diffuse infiltrating (38% of cases). Our histologic classification includes 4 categories: (a) adenocarcinoma (70% of cases), (b) round cell carcinoma, (c) mucinous carcinoma, and (d) rare forms of carcinoma (squamous, adenoacanthoma, carcinosarcoma, collision tumor, ductogenic carcinoma). These main histologie categories are subdivided according to the degree of differentiation of each tumor. From 81 to 86% of gastric cancers arise in the pyloric region. Multifocal origin of stomach cancers occurs in about 10% of cases. The prognosis of gastric cancer depends on the extent of spread at the time of treatment. Intramural propagation beyond the grossly apparent margins is common, and extension into the duodenum and esophagus from lesions in the pylorus and cardia regularly occurs. Approximately 50% of gastric cancers infiltrate neighboring organs. Metastases to regional lymph nodes, particularly in the lesser curvature drainage area, occur in over half of the cases. Blood‐borne spread, to the liver and beyond, and peritoneal seeding are common.

Statistics of postmortem examinations and retrospective long-term observations indicate a disposition of the resected stomach for the development of a primary carcinoma. The risk to develop a carcinoma after resection for peptic ulcer shows a two- to three-fold increase compared to gastric cancer mortality of the normal population. Because of the lack of typical symptoms for gastric stump cancer surgical treatment will occur late and the 5-years survival rate is only 1.38%. The cause of the increased cancer risk after gastric resection seems to be the atrophic gastritis in the gastric remnant which is found after a few years.
