Lymphatic and Mixed Malformations

Abstract: Lymphatic and mixed malformations are rare and variable in presentation. They arise due to errors in vascular and lymphatic formation during early embryonic development. This leads to persistent infiltration of lymph fluid into soft tissues and causes a locally invasive mass with pathologic sequelae. Departing from historically descriptive terminology, such as ''cystic hygroma,'' lymphatic malformations are now categorized as macrocystic, microcystic, or mixed lesions, based on size. Advances in imaging modali… Show more

“…Lymphatic malformations (LMs) are common diseases in the pediatric population. Macrocystic, microcystic, and mixed-type are the three categories of LMs, classified based on the diameter of cysts, with cutoffs of 1 cm or 2 cm ( 5 ). The head and neck are the most commonly affected areas, accounting for approximately 50% of all LMs ( 6 ).…”

“…The main methods include surgery, sclerotherapy, oral medication therapy, and observative treatment ( 10 ). Management requires the coordinated efforts of specialists in both medical and surgical fields and should be tailored to the size, location, symptoms, and category of the lesions ( 5 ).…”

“…Microcystic LMs present with cysts <1–2 cm and often show a more aggressive growth pattern, whereas mixed cystic LMs show a combination of both and are more frequent in central parts of the body. Lymphatic malformations with microcystic components are more difficult to treat with high recurrence rates after attempted resection (>20%) 162 . Sclerotherapy is a treatment option further discussed below.…”

“…Lymphatic malformations with microcystic components are more difficult to treat with high recurrence rates after attempted resection (>20%). 162 Sclerotherapy is a treatment option further discussed below. Because of high recurrence rates after local therapy and even possible worsening due to induction of further growth, medical therapy of LMs is gaining importance (eg, sirolimus).…”
