Luteinizing Hormone Releasing Hormone Agonist for Contraception in Breast Feeding Women*

Abstract: During the period of lactation there is a need for a reliable method of contraception since the suppressive effects of lactation on ovulation decline as the duration of breastfeeding is decreased. The aim of this study was to establish that chronic treatment with a LHRH agonist would prevent ovulation throughout the period of lactation and to evaluate the effects of the treatment on estrogen production, bleeding patterns, and nursing practice. Starting 6 weeks postpartum, nine mothers took 300 micrograms LHRH … Show more

“…In a similar earlier work, Fraser et al [5] show a total ovulation inhibition in postpartum women employing a daily dose of 300 txg buserelin. Since no pregnancies were registered during the study, we can conclude that ovulation was inhibited in both treated groups.…”

“…In recent years several potent GnRH analogs have become available for female contraception, and one of them, buserelin, has been studied in lactating women and has shown good results [5,6]. However, the metabolic effects of treatment have not been investigated in these trials.…”

“…When it is administered daily by nasal spray for long periods of time, it can successfully prevent ovulation [6][7][8][9][10]. Chronic administration results in pituitary desensitization, disruption of the normal pulsatile release of the gonadotropins, change in the LH/FSH ratio, and impairment of the estrogeninduced LH surge so that ovulation fails to occur [11].…”

“…It has already been established that ovulation is inhibited during breastfeeding, and that small amounts of agonist in the breast milk have no effect on LH secretion in the infant as a result of an extremely low degree of oral absorption, estimated to be <0.1% [11,12].…”
