Long-term safety and efficacy of recombinant activated factor VII (NovoSeven) in haemophilia patients with inhibitors: interim post marketing study analysis of 5 patient exposure years

遺伝子組換え活性型凝固第 VII 因子製剤(rFVIIa：注射用ノボセブン ®)は，インヒビターを保有する血友病 A および B 患者の止血を目的に開発されたバイパス止血製剤である．海外では rFVIIa 高用量単回投与の臨床研究が複数行われ，2007 年 EU において高用量単回投与法が正式に承認された．今回，国内で行われた，高用量(270 μg / kg)単回投与に関する医師主導型臨床研究の結果を報告する．第 I 相試験では rFVIIa 270 μg / kg 単回投与に対する安全性と凝血学的薬理作用を検証した．さらに，第 II 相試験では 270 μg / kg の高用量単回投与と 90 μg / kg の標準用量 3 回投与における有効性と安全性の差異を，無作為割り付けクロスオーバー方式で検討した．その結果，第 I 相試験では 270 μg / kg 単回投与の安全性と薬理作用が裏付けられた．さらに第Ⅱ相試験において 270 μg / kg 単回投与法は，標準投与法に比べ，同等以上の有効性を示した．また，安全性においても，高用量単回投与法で新たに問題となる事象は認められなかった．以上の結果から，患者・家族の負担を軽減し，QOL の改善につながる高用量単回投与法を我が国においても普及させるべきである．

診断・治療・技術講座遺伝子組換え活性型凝固第 VII 因子製剤 (rFVIIa：注射用ノボセブン ® ，ノボノルディスクファーマ株式会社)は，凝固第 VIII 因子あるいは第 IX 因子に対する抑制抗体(インヒビター)を保有する血友病 A および B 患者の止血治療を目的に開発されたバイパス製剤である．我が国では 2000 年 3 月に承認され，現在広く臨床使用されている．本稿では rFVIIa の作用機序，血友病領域における現在の使用方法，および新しい用法について紹介する．また現在日本では，rFVIIa における承認上の適応疾患は先天性血友病及び後天性血友病患者に限定されているが，欧米などでインヒビター保有血友病以外に承認されている rFVIIa の適応症やその他の類縁疾患に対する止血治療の可能性についても紹介する．最後に，後継製品として開発されている次世代の rFVIIa についても言及する． 1 rFVIIa 1 -3 生理的な止血機構( 1a)は，血管傷害部位の内皮細胞上に発現した組織因子(tissue factor：TF) ，血管内皮下組織中(線維芽細胞) に存在する組織因子，あるいは活性化された単球上の組織因子に循環中の活性型第 VII 因子 (VIIa)が結合することから開始される．この TF / VIIa 複合体は細胞膜上で基質である第 X 因子を活性化する．活性型第 X 因子(Xa)は第 V 因子，プロトロンビン(第 II 因子)と複合体を形成し，小量のトロンビンが生成される．このトロンビンはフィブリノゲンを活性化させるには不十分だが，第 VIII 因子，第 V 因子および血小板を活性化することができる．以上が反応の初期相(initiation phase)であり，凝固因子活性化の環境を与える．活性化血小板膜上ではさらに凝固因子活性化の増幅が進行する．上記の組織因子発現細胞上で活性化した第 IX 因子(IXa)は，活性化血小板膜上で VIIIa および第 X 因子と複合体(Xase complex)を形遺伝子組換え活性型第 VII 因子製剤による出血性疾患の治療：現状から展望まで桑原光弘 1＊，新井盛大 2＊
