Long-term airway space changes after mandibular setback using bilateral sagittal split osteotomy

Introduction Pharynx is a muscular organ with is sustained by craniofacial bones. It is divided into nasal, oral and hipopharynx, and can be considered as a tube that serves both respiratory and digestive systems. Its anatomical morphology permits that factors facilitate its obstruction, leading to the sleep apnea syndrome. One of the treatment consists of surgical mandibular advancement, increasing pharyngeal dimensions. The aim of this study was evaluate the cephalometric changes in the pharyngeal airway space after orthognathic surgery procedures for correction of mandibular prognathism. Materials and Methods Pre and post-operative cephalometric analysis was performed on 19 patients submitted to mandibular setback by mandibular bilateral sagittal split osteotomy associated with maxillary advancement by Le Fort I osteotomy, using the Dolphin Imaging 10.0 software. Results Results did not reveal statistically significant changes in the upper (nasopharyngeal), middle (oropharyngeal) and lower (hypopharyngeal) airway spaces, but showed increase in the nasal pharynx due the maxillary advancement and a lower position of the hyoid bone due the mandibular setback.Discussion Maxillomandibular orthognathic surgery for correction of mandibular prognathism does not seem to statistically significantly change the pharyngeal airway space, but it increases the maxillary airway. Conclusion It seems to be important to consider the double jaw surgery in cases of mandibular prognathism, aiming prevention of a possible reduction of whole upper airway.

Resumo Introdução:A configuração e as dimensões da via aérea superior são determinadas pelas estruturas anatômicas como: tecidos moles, musculatura e esqueleto craniofacial, que compõem ou circundam a faringe. As alterações anatômicas dos tecidos moles e/ou do esqueleto craniofacial poderão tornar a via aérea superior (VAS) mais estreita. A cirurgia ortognática, que é utilizada na correção das deformidades dento-esqueléticas, leva também a alterações na VAS. Objetivo: Em vista dos fatos apresentados, o presente artigo tem como objetivo realizar uma revisão da literatura sobre as alterações da via aérea superior em pacientes classe III submetidos à cirurgia ortognática. Metodologia: Foram selecionados artigos originais e de revisão publicados entre 1990 e 2010, em duas bases de dados bibliográficos -Literatura Internacional em Ciências da Saúde (Pubmed) e Porta de Periódicos CAPES. Resultados: Foram selecionados trinta e nove (39) artigos para a escrita dessa revisão. Conclusão: A via aérea superior e a deformidade dento-esquelética classe III devem ser avaliadas cuidadosamente antes da cirurgia ortognática e sempre que o planejamento cirúrgico permitir deve-se preferir os avanços maxilares aos recuos mandibulares. Palavras INTRODUÇÃOO tratamento ortodôntico-cirúrgico é cada vez mais utilizado no tratamento de pacientes portadores de deformidade dento-esquelética. Quando essa modalidade de tratamento é adotada, a estética e função devem ser cuidadosamente avaliadas. A deformidade dento--esquelética classe III é resultado de um prognatismo mandibular e/ou uma deficiência maxilar. Historicamente, a correção cirúrgica dessa deformidade dava-se apenas pela cirurgia de recuo mandibular. Sabe-se que o excesso ântero-posterior da mandíbula isoladamente ocorre somente em 20-25% dos casos. Atualmente as cirurgias bimaxilares ou de avanço maxilar isolado têm sido utilizadas com mais frequência para o tratamento de pacientes com deformidades classe III. Na última década as cirurgias de recuo mandibular têm diminuído e passaram a representar somente 10% dos procedimentos, enquanto que as cirurgias bimaxilares e de avanço maxilar são realizadas no restante dos pacientes 1,2,3,4 . The purpose of this study was to investigate volume changes in posterior airway space (PAS) after bilateral mandibular advancement surgery. Measurements were taken based on three-dimensional (3D) records available for a large and homogeneous cohort of patients. Pre- and postoperative CBCT scans of 102 adult patients with Class II dysgnathia were visualized and analyzed using 3D software (Mimics® Innovation Suite 14.1; Materialise, Leuven, Belgium). The PAS was divided into three segments by three planes parallel and one plane perpendicular to the Frankfort horizontal plane. Total volume, partial volumes, and cross-sectional areas were calculated from the pre- and postoperative scans. Dahlberg coefficients were obtained to verify each parameter for the measurements' reliability. The statistical significance of the changes observed was analyzed by Wilcoxon's rank-sum test. Highly significant (p=0.000) increases in total posterior airway volume (+32.0%) were noted as an effect of mandibular advancement surgery, amounting to 45.6% in the lower PAS third compared to 38.8% in the middle and 12.5% in the upper PAS third. We also obtained highly significant (p=0.000) increases in all the cross-sectional areas investigated, amounting to 48.5% on the soft-palate level compared to 21.6% on the level of the epiglottis tip, and 14.6% on the hard-palate level. These results demonstrate that bilateral mandibular advancement surgery in Class II-Patients leads to significant increases in PAS volume and significant widening of the narrower sites inside the pharynx.
