Resumo Objetivo O objetivo deste estudo foi avaliar o perfil nociceptivo e o uso de analgésicos em pacientes submetidos à cirurgia de reparo do manguito rotador. Além disso, os limiares nociceptivos e a integridade do sistema inibidor descendente, o catastrofismo da dor e a prevalência de dor nociceptiva ou neuropática também foram analisados. Métodos Este estudo foi aprovado pelo Comitê de Ética da Universidade La Salle (1.325.433/2015). Quarenta pacientes (maiores de 18 anos) submetidos à cirurgia de reparo do manguito rotador (divididos entre aqueles com lesões pequenas e grandes) participaram do estudo. Os instrumentos utilizados foram o Questionário Sociodemográfico, a Escala Funcional de Dor, a Escala Visual Análoga (EVA), o Teste Sensorial Quantitativo (QST) e a Tarefa de Modulação Condicionada da Dor (CPM). Resultados Os pacientes apresentaram diferenças significativas nos limiares de dor e QST de calor (teste t de amostras independentes) e qualidade do sono, humor e ansiedade (teste t pareado) nos grupos pré-operatórios. Houve uma correlação significativa entre CPM pré-operatória e EVA pós-operatória (correlação de Pearson). Observou-se que, no período pré-operatório, 38 pacientes utilizavam analgésico de forma contínua. Além disso, no período pós-operatório, o uso de opioides foi maior nos pacientes com lesões pequenas (13 pacientes) em comparação àqueles com lesões grandes (nove pacientes). Conclusão Os pacientes com lesão do manguito rotador não apresentaram alterações no sistema inibidor descendente, mas sim alterações nos limiares de dor, o que pode interferir no período pós-operatório e estar relacionado ao consumo de analgésicos.

Long-term post-operating functionality of rotator cuff repair

OBJECTIVE: The purpose of this study was to assess, late, the functionality of patients undergoing surgery to repair the rotator cuff. METHODS: Cross-sectional observational study performed at the UNISA Orthopedics and Rheumatology clinic, approved by the ethics committee followed by CAAE 07824918.0.0000.0081. The sample consisted of individuals who underwent rotator cuff repair for at least two years and who have already undergone postoperative rehabilitation. For the evaluation, a specific self-report questionnaire was applied to assess rotator cuff dysfunctions, the WORC, and the FIT-HaNSA test, a functional test that consists of performing 3 tasks that require the elevation of the upper limb. RESULTS: A convenience sample of 9 individuals participated in this study, of both genders (5 men and 4 women) with a mean age of 54.1 years (± 3.4 years). The results in all the tasks of the FIT-HaNSA test, showed a greater functional deficit in the operated limb compared to the contralateral limb. In the WORC questionnaire, the sample subjects showed better scores as the post-surgical time increased. CONCLUSION: Although sample participants demonstrate better results in WORC over time, in the functionality test it was observed that patients undergoing rotator cuff repair have a long-term deficit in functionality, even after undergoing rehabilitation.

Effectiveness of ivermectin for COVID-19: A systematic review

OBJECTIVE: To evaluate the efficacy and safety of ivermectin in the prevention and treatment of COVID-19. METHODS: This is a systematic review of randomized clinical trials. We searched the electronic databases PubMed (1966-2021), EMBASE (1974-2021) and Clinical Trials (2021) and two evidence megabusers: Turning Research Into Practice (TRIP) database (2021) and Epstemonikos (2021). There was no geographic or language restriction, using DeCS descriptors and terms (Health Sciences Descriptors). The synthesis method involved the combination of similar studies in a narrative review. RESULTS: 463 citations were identified, and 2 studies were included, following the inclusion and exclusion criteria. Both studies showed very low quality and reduced sampling. CONCLUSION: The studies completed and published to date do not support the use of ivermectin in the prevention or treatment of COVID-19. It is suggested to carry out new quality clinical trials to elucidate the issue.
