Late stent thrombosis after implantation of a sirolimus‐eluting stent

Abstract: Late stent thrombosis in the era of routine high-pressure stent deployment and combined antiplatelet therapy with thienopyridines and aspirin has become a rare but feared complication. We describe a patient with acute myocardial infarction due to late stent thrombosis 6 weeks after deployment of a sirolimus-eluting stent and 2 weeks after the discontinuation of clopidogrel. This is the first report of late thrombosis of a sirolimus-eluting stent.

“…When the criteria were broadened to include patients with PCI, stroke, PAD and STEMI, most of the retrieved data were for patients who had undergone PCI. Of these: five RCTs, 50-54 two observational cohorts, 38,55 nine case series, 56-64 and 33 case reports across 17 publications [65][66][67][68][69][70][71][72][73][74][75][76][77][78][79][80][81] investigated clopidogrel and ticlopidine therapy. Only two case series 82,83 and two case reports in a single publication 84 were identified for patients who had not undergone PCI.…”

“…Although subacute thrombosis has received great attention related to drug-eluting stents, it must be remembered that it also occurred with bare metal stents [64,65,66,67,68,69,70,71]. Registry studies documented an incidence of 0.6–0.9% of patients with bare metal stents [64].…”

“…3 Stent thrombosis usually results in ST-segment elevation myocardial infarction (MI) or death, 4 and in the era of DESs, late stent thrombosis may be a possible limitation of the procedure. In the international reports of late angiographic stent thrombosis (LAST), [5][6][7][8][9][10] clopidogrel was used as the antiplatelet therapy in most patients/studies. As antithrombotic therapies have a major impact on angiographic stent thrombosis after BMS or DES implantation, 11,12 we wanted to evaluate the incidence, promoting factors and outcomes of LAST in Japan where clopidogrel has not been approved.…”
