Itch and pain by lesion morphology in hidradenitis suppurativa patients

Abstract: was difficult in the present case; melanotrichoblastoma with seborrheic keratosis was finally diagnosed on the basis of the pathological findings. Although pathological findings are the gold standard for the diagnosis of melanotrichoblastoma, we believe that dermoscopic features might help in the diagnosis of melanotrichoblastoma even in cases of coexistence with seborrheic keratosis.

“…181 Although one decade ago HS was not considered a pruritic disease, itch is also a common HS-associated symptom (62.1%-77.5% of reporting patients) that adversely affects patients' quality of life. [181][182][183] Of note, the co-occurrence rate of pain and itch in one location was surprisingly high, as reported by 59.5%-74.9% of patients. 181,182 It could be assumed that both, acute pain observed during flare- inflammatory mediators into the central nervous system.…”

“…Prevalence of pruritus in symptomatic HS patients has been estimated from 41.7% to 77.5% of patients, with higher intensity scores being associated with severe Hurley 3 disease, extensive disease and pain [30]. In most reports, pain and pruritus intensity scores correlate positively with each other and both symptoms are perceived spatially in the same disease body region in 60-74.9% of patient [31,32]. On histology, eosinophilic and perineural inflammation as well as abnormal mast cell innervation has been shown in both peri-lesional and lesion skin of HS patients, pointing at a potential anatomical correlate of "neuropathic" pain [33].…”
