Is It Inflammatory Bowel Disease Flare or Pediatric Inflammatory Multisystem Syndrome Temporally Associated with COVID-19?

Abstract: Background: Pediatric inflammatory multisystem syndrome temporally associated with COVID-19/multi-system inflammatory syndrome in children (PIMS-TS/MIS-C) is a potentially life-threatening complication of SARS-CoV-2 infection in children. Gastrointestinal manifestations are prominent in children with PIMS-TS/MIS-C. Thus, it is challenging to differentiate this condition from an exacerbation of inflammatory bowel disease (IBD). We aimed to present the clinical characteristics, and diagnostic and therapeutic dif… Show more

“…Interestingly, there have been reported MIS-C cases where abdominal imaging reveals inflammation of the bowel wall in the distal ileum and mesenteric fat, a usual finding in IBD cases [ 33 ]. The overlapping symptoms of PIBD exacerbation and MIS-C pose a challenge for accurate diagnosis and decision on the appropriate treatment [ 23 , 33 , 34 , 35 ]. Physicians should be aware of the clinical presentations of both entities in order to provide the best available care to their patients.…”

“…In the 8 cases of KD with appendicitis published so far, histological findings showed found focal inflammation with neutrophilic and eosinophilic infiltration in one patient and transmural inflammation and arteritis in another one ( 33 ). In case of persistent postoperative fever, especially if other systemic signs appear, the diagnosis should be reconsidered, with the awareness that KD tends to affect younger patients than MIS-C ( 34 ).…”

“…Podstępne i ciężkie objawy żołądkowo-jelitowe PIMS mogą naśladować nagłe przypadki chirurgiczne w obrębie jamy brzusznej, w tym ostre zapalenie wyrostka robaczkowego, infekcje żołądkowo-jelitowe lub nieswoiste zapalenie jelit. [5] Typowo u chorych obserwujemy ból brzucha, biegunkę oraz wymioty. W badaniach laboratoryjnych notujemy podwyższone enzymy wątrobowe (33,3%) [6].…”
