Veel (psychologische) behandelingen van psychische klachten verdienen tegenwoordig het predikaat 'evidence based'. Voor tal van stoornissen zijn behandelprotocollen ontwikkeld en onderzocht en de richtlijnontwikkeling in de GGZ heeft een hoge vlucht genomen. Sommige (meestal langdurige) behandelvormen zijn hierdoor duidelijk onder druk komen te staan. Bij deze ontwikkeling naar een meer evidence based klinische praktijk is het van belang onderscheid te maken in werkzaamheid (efficacy), doeltreffendheid (effectiveness) en doelmatigheid (efficiency), zie Jaspers (2001). Niet alleen sommige behandelvormen zijn door de nadruk op evidence based werken aan kritiek blootgesteld, ook het nut en de noodzaak van (psycho)diagnostiek is op verschillende manieren onderwerp van discussie geweest.Veel behandelprotocollen zijn ontwikkeld door cognitief gedragstherapeuten. Binnen dit referentiekader is ook altijd veel aandacht besteed aan een gedegen, op het individu toegesneden probleemanalyse, op basis waarvan het behandelplan en de toe te passen behandelmethoden en -technieken dienen te worden gekozen. Dit maatwerk is in de praktijk veelal vervangen door het 'confectiepak' van het behandelprotocol, dat op basis van een classificatie van de stoornis op As 1 van de DSM in veel gevallen als toereikend wordt beschouwd. Sinds de opkomst van de behandelprotocollen zijn in gedragstherapeutische kringen dan ook vraagtekens gezet bij de noodzaak van het opstellen van geïndividualiseerde probleemanalyses in de vorm van een holistische theorie, functieanalyses en betekenisanalyses. Op een recente regionale bijeenkomst van gedragstherapeuten in Groningen ging Theo Bouman nog een stapje verder door de vraag te stellen hoe evidence based deze diagnostische procedures eigenlijk zijn. Hij constateerde dat de gedragstherapeutische literatuur, in het bijzonder de leerboeken voor gedragstherapeuten, vooral prescriptief zijn. Hierin wordt uitvoerig beschreven hoe de analyses dienen te worden gemaakt. Onderzoek naar betrouwbaarheid en validiteit van dit diagnostisch proces ontbreekt echter.Op het eerste gezicht lijkt de situatie wat betreft psychologische tests aanzienlijk beter. Psychometrisch onderzoek naar tests en vragenlijsten krijgt meestal veel aandacht, ook al betekent dit niet dat de wijze waarop de tests in het diagnostisch proces worden gebruikt altijd betrouwbaar en valide is. Het meest schrijnende voorbeeld hiervan is de profielinterpretatie van de Nederlandse verkorte MMPI, de NVM, ook wel bekend als de methode Eurelings-Bontekoe / Snellen. Over de onbetrouwbaarheid van deze methode brandde enkele jaren geleden de discussie los (Jaspers, 2005;Smid & Kamphuis, 2005;Eurelings-Bontekoe & Snellen, 2005), die recent in het voordeel van de tegenstanders van de methode lijkt te zijn beslecht. Brogtrop, Lindeborn en Ingehoven (2008) stelden vast dat de overeenstemming tussen twee methoden om de zogenaamde persoonlijkheidsorganisatie vast te stellen, het structureel interview van Kernberg en de profielinterpretatie van de NVM, ronduit slecht ...

InleidingVan der Heijden, Derksen en Egger (2008) pleiten in hun bijdrage voor een integratie tussen diagnostiek en behandeling. Zij zien psychologisch testonderzoek als een methode om objectieve informatie te verzamelen die een unieke en waardevolle bijdrage kan leveren aan het inzicht in de cliënt en diens mogelijkheden en daarmee rechtstreeks aan het therapeutisch proces. Zij beginnen hun bijdrage met de stelling dat 'echte' psychotherapeuten niet testen; hun testvaardigheden zouden verouderd, versleten of anderszins achteruitgegaan zijn. Deze stellingname vraagt om een reactie. Want zelfs als deze bewering, waar zij overigens geen evidentie voor aanvoeren, waar zou zijn, dan nog is het de vraag of testdiagnostiek, in welke vorm dan ook, wel zo'n waardevolle bijdrage vormt aan het werk van psychotherapeuten als Van der Heijden et al. suggereren. Cruciaal hierbij lijkt mij de vraag of testdiagnostiek een meerwaarde heeft voor het effect van de behandeling. Alvorens uitgebreider in te gaan op die vraag, maak ik enkele kanttekeningen bij de verschillende thema's die in hun betoog aan de orde komen.
