Objective: to understand nursing workers' experiences with the nursing care provided to elderly individuals with dementia living in a long-term care facility. Method: comprehensive qualitative study conducted in a long-term care facility located in Joinville, Santa Catarina, Brazil, from July to September 2018. Thirteen nursing technicians and one nurse participated. Data were collected using questionnaires, field diaries, individual interviews, and participatory observation. Thematic content analysis was used to interpret content. Results: three themes emerged from data analysis: "Knowledge acquired with practice and gap existing in the care provided to institutionalized elderly individuals with dementia"; "Individualized care provided to elderly individuals and bonding"; and "Conflicts the nursing staff faces when reconciling care needs, the time available, and the facility's routines". These themes originated the central theme: Meanings assigned to the nursing care provided to institutionalized elderly individuals with dementia. By sharing information regarding care actions that obtained positive results, the staff perceived the importance of an individualized approach and of establishing bonds as a therapeutic process. Gaps were identified in the professionals' knowledge regarding how to provide hygiene care when individuals become aggressive, agitated, or resist care. Conclusion: health workers' experience is a source of evidence for evidence-based practice and is also essential to fill in existing gaps between knowledge and care delivery.

Rheumatoid arthritis: profile of patients and burden of caregivers

Objective: to describe the profile of patients with Rheumatoid Arthritis (RA) and their caregivers receiving care at the Rheumatology Outpatient Clinic of a teaching hospital, and evaluate the burden of the caregivers. Method: a cross-sectional study was performed with 41 patients with RA and their caregivers using a questionnaire to identify sociodemographic variables; the Burden Interview Scale and the Stanford Health Assessment Questionnaire. Descriptive analyzes and comparison between clinical-demographic variables and the functional status of patients were performed and the correlation between sociodemographic variables and levels of burden of caregivers was tested. Results: there was a prevalence of female patients (87.8%); a mean age of 64.4 years (±12.9); a mean time for the diagnosis of RA of 13.5 years (±8.5), a prevalence of moderate disability (39.0%); lower disability in the Hygiene domain (1.6; ±0.5) and greater disability in the Other Activities of Daily Life (2.1; ±0.6), Reach (2.0; ±0.7) and Grip (2.0; ±0.7) domains. The caregivers were women (73.2%); aged between 17 and 81 years (mean: 46.8; ±15.1); with a high school education (41.4%). The degree of kinship was 56.2% offspring and 36.6% spouses. Eighteen (44.0%) caregivers suffered burden, nine (22.0%) of whom had mild burden and nine (22.0%) of whom suffered intense burden. There was a higher incidence of intense burden among spouses (12.2%) and mild burden among children (12.2%). Conclusion: the low occurrence of burden among caregivers may be related to the profile of the patients, who presented good levels of independence for self-care. The profile of caregivers and the prevalence of overburdened spouses and offspring shows the need and importance of the implementation of caregiver training by health service professionals to improve care for RA patients.

Introdução: As condições de vida do idoso em instituições de longa permanência tem sido objeto de pesquisas visto à falta de suporte, sobretudo de natureza afetiva e de apoio familiar. Objetivo: Este estudo teve por objetivo conhecer as características socioeconômicas e de proximidade familiar dos idosos que vivem em uma instituição de longa permanência localizada em Joinville, Santa Catarina. Materiais e Métodos: Trata-se de um estudo descritivo de caráter exploratório que foi realizado com dez idosos. Os dados foram coletados por meio de um questionário semiestruturado. As variáveis investigadas consideraram as características socioeconômicas, aspectos relacionados à saúde, à família e a instituição de longa permanência. Resultados: A idade dos idosos variou entre 66 a mais de 80 anos. A maioria era do sexo masculino, possuía ensino médio completo e referiu problemas de saúde. O tempo de institucionalização dos idosos variou de 4 meses a 10 anos e motivo mais frequente para a institucionalização foi a dependência física. Mais da metade dos entrevistados tiveram resposta positivas sobre gostar de estar na Instituição de Longa Permanência. Conclusões: A institucionalização do idoso pode ser geradora de sofrimento, mas também pode constituir-se um ambiente de conformação de vínculos afetivos. O idoso atribui considerável importância aos cuidados, o que impõe aos profissionais uma atenção que abranja os vários aspectos da vida da pessoa envelhecida. Em razão do envelhecimento populacional, seria desejável que fossem realizados estudos que aprofundassem a analise de outros fatores que poderiam estar contribuindo para qualidade de vida dos idosos institucionalizados.
