Intervenção fonoaudiológica em recém-nascido pré-termo com gastrosquise

A prematuridade é um dos fatores mais incidentes no ambiente hospitalar infantil. O trabalho fonoaudiológico realizado em hospitais com foco no recém-nascido prematuro tem conquistado cada vez mais espaço e apresenta como proposta principal a detecção de problemas relacionados à deglutição, proporcionando recurso aos neonatos que fazem uso de via alternativa de alimentação, devido à imaturidade orgânica. O uso de técnicas para maturação do sistema sensório-motor-oral vem sendo explorado a fim de que o recém-nascido pré-termo realize transição, mais segura e em menor tempo, para alimentação plena por via oral. O objetivo deste estudo é realizar revisão sistemática da literatura nacional e internacional a fim de elencar diferentes técnicas e elucidar sobre a eficácia das mesmas, quanto à maturação do sistema sensório-motor-oral em neonatos prematuros que permanecem em ambiente hospitalar. Segundo a literatura, a técnica mais utilizada no Brasil é a sucção não nutritiva com dedo mínimo enluvado ou chupeta ortodôntica. Em outros países, as técnicas mais citadas foram a sucção não nutritiva com chupeta seguida da estimulação do sistema sensório-motor-oral. Tanto as técnicas utilizadas no Brasil quanto no exterior têm refletido positivamente no desenvolvimento das funções orais de neonatos. Levando em conta a necessidade de promoção do aleitamento materno exclusivo, publicações recentes evidenciaram alarmante e preocupante percentual de uso de bicos sintéticos para estimulação em unidades de terapia intensiva neonatal.

This assumption, the human being has a wide behavioral repertoire at birth and ABSTRACT Purpose: to investigate the existence of the alimentation system in premature newborns in response to gustatory stimulation. Methods: experimental, analytical, double-blind study. 90 premature newborns of a public maternity in Sergipe took part in the test which was filmed and divided into three parts of five minutes. In the first and last, there was no stimulus; in the second, the gustatory stimulation was applied and the newborn children were divided into two groups (water or sucrose). We studied the specific behaviors suction right and left hands, tongue protrusion and suction movements in behavioral states deep sleep, light sleep, drowsy, restless / irritable and crying. In the statistical analysis of the population, average, standard deviation and prevalence studies were performed. We used the nonparametric Mann-Whitney test to compare averages. The Spearman test observed correlation between behavioral states at each time of the test. The p value was significant when less than 0.05. Results: independent of the given stimulus, the correlation increased in all specific behaviors. Comparing the groups separately, after stimulation, we observed an increase in correlation in right hand suction and tongue protrusion for both. The same happened in suction, except for the agitated/irritated state. After stimulation, there was a higher correlation to the behavior of left hand suction in the sucrose group when compared to water. The results show that gustatory stimuli may contribute to the readiness to feed this population. Conclusions: it was found in premature newborns an increased in correlation for the specific behaviors related to the alimentation system after oral stimulation, which envisions the possibility of gustatory stimulation be used for activating a alimentation system in premature newborns.

Objective: to identify the scientific evidence about the abilities of premature neonates in Neonatal Intensive Care Unit, to oral feeding initiation. Methods: integrative review, held in the databases Scopus, Cinahl, Medline, Lilacs, Science Direct and Web of Science, not controlled and with no timeframe, using the key words: Recém-Nascido Prematuro/Premature, Infant; Comportamento Alimentar/Feeding Behavior and Aleitamento Materno/ Breast Feeding, crossing them with the AND operator. Inclusion criteria: full available in Portuguese, English and Spanish, without year limitation; and exclusion: articles that did not answer the research question, repeated and from other scientific sources. Results: the scientific productions revealed that the sensory-motor-oral stimulation, non-nutritious sucking and evaluation of hemodynamic parameters favor the transition from probe-oral to breastfeeding. Conclusion: we identified evidence that prove that the use of the stimulation and skills development facilitate, in less time, the beginning of oral feeding. Resultados: as produções científicas revelaram que a estimulação sensório-motora-oral, sucção não nutritiva e avaliação de parâmetros hemodinâmicos favorecem a transição sonda-oral para o aleitamento materno. Conclusão: foram identificadas evidências que comprovam que o uso da estimulação e o desenvolvimento das habilidades facilitam, em menor tempo, o início da alimentação oral.
