There has been tremendous growth in the application of the Internet of Things (IoT) in our daily lives. Yet with this growth has come numerous security concerns and privacy challenges for both the users and the systems. Smart devices have many uses in a healthcare system, e.g. collecting and reporting patient data and controlling the administration of treatment. In this paper, we address the specific security issue of access control for smart healthcare systems and the protection of smart things from unauthorised access in such large scale systems. Commonly used access control approaches e.g. Role-Based Access Control (RBAC), AttributeBased Access Control (ABAC) and Capability-Based Access Control (CapBAC) do not, in isolation, provide a complete solution for securing access to IoT-enabled smart healthcare devices. They may, for example, require an overly-centralised solution or an unmanageably large policy base. We propose a novel access control architecture which improves policy management by reducing the required number of authentication policies in a large-scale healthcare system while providing fine-grained access control. The devised access control model employs attributes, roles and capabilities. We apply attributes for role membership assignment and in permission evaluation. Membership of roles grants capabilities. The capabilities which are issued may be parameterised based on attributes of the user and are then used to access specific services provided by things. We also provide a formal specification of the model and a description of its implementation and demonstrate its application through different use-case scenarios. The evaluation results of core functionality of our architecture are provided with the practical testbed experiments.

ResumoA obesidade, atualmente, é um dos problemas mais importantes de Saúde Pública no Brasil e em diversos países no mundo porque pode ocasionar diversas doenças (cardíacas, diabetes, alteração nas taxas do colesterol e outras). O sucesso do tratamento das doenças crônicas, em especial a obesidade, depende fortemente da participação e envolvimento do indivíduo enquanto sujeito ativo de seu tratamento. O uso de mundos virtuais 3D foi estudado no presente trabalho como estratégia de apoio com vistas a oferecer ao usuário a sensação de "estar presente" num contexto especialmente preparado para promover a motivação e permitir a interação com objetos 3D. Estes elementos têm potencial para motivar e influenciar o indivíduo participante do programa. Este artigo relata um estudo que investigou o impacto da combinação de mundos virtuais 3D com Internet das Coisas e dispositivos móveis como elementos capazes de promover a motivação de sujeitos envolvidos em um programa de controle de obesidade.O trabalho foi apoiado na teoria da autodeterminação usando estratégias de motivação extrínseca, com vistas a alcançar mudança de comportamento e em consequência, ganhos em melhoria da saúde e qualidade de vida.Palavras chaves: mundos virtuais 3D; obesidade; motivação; autodeterminação; sensores. AbstractObesity is currently one of the most important public health problems in Brazil and in several countries in the world because it can cause various diseases (heart, diabetes, abnormal cholesterol and other fees). Successful treatment of chronic diseases, especially obesity, strongly depends on the participation and involvement of the individual as an active subject of their treatment. The use of 3D virtual worlds has been studied in this work as a support strategy in order to offer the user the feeling of "being there" in an environment specially prepared to promote motivation and allow interaction with 3D objects. These elements have the potential to motivate and influence the individual participant of the program. This paper reports a study that investigated the impact of the combination of 3D virtual worlds with Internet of Things and mobile devices as elements capable of promoting the motivation of individuals involved in an obesity management program. The work was supported in selfdetermination theory using extrinsic motivation strategies, in order to achieve behavior change and as a result, gains in improving health and quality of life.

The Internet of Things (IoT) describes the future where every day physical objects will be connected to the internet and be able to identify themselves to other devices. IoT is a new revolution of the Internet and It will effect in a large number of applications such as smart living, smart home, healthcare systems, smart manufacturing, environment monitoring, and smart logistics. This paper provides integration, summarizes and surveys some of the security techniques especially hybrid techniques that can be applied with healthcare applications in IoT environment.
