SUMMARYWe describe and analyze 30 veno-venous hemofiltration procedures in 22 adult patients with diagnosis of acute renal failure, between September 1991 and July 1992. We used Hospal Polycrylonitrile AN-69 filters (1.0, 1.2 and 1.7m2 ) and Duodial machines, designed and made in Peru. The procedure efficiency was evaluated through low weight solute (urea, creatinine, uric acid) clearances and the correlation between the basal concentration of seric electrolytes (sodium, calcium and phosphorus) and the deltas obtained at the end of the sessions. The clearances were: urea 62.6 ±17.5, creatinine 56.7±23.4 and uric acid 47.5±15.6 ml/min. The solutes clearances improved by increasing the venous resistence in the extracorporal circuit (r=0.63, p<0.005). the electrolyte imbalances were adecuately corrected: natremia r=0.54, p<0.05, kalemia r=0.65, p<0.05, calcemia r=0.76, p<0.05, phosphatemia r=0.27, p ns. The hemodynamic evolution was studied through the correlation between the sistolic and diastolic basal pressures and the deltas at the end of the session. Our dates confirm the low risk of hemodynamic disequilibrium in hemofiltration. The sistolic blood pressure correlation was -0.62 (p<0.05) and -0.54 (p<0.05) for the diastolic pressure. Low costs and efficiency make possible to set up descentralized renal units where hemofiltration could be used as substitution therapy for patients with renal failure. RESUMENDescribimos y analizamos la experiencia obtenida en treinta procedimientos de Hemofiltración veno-venosa (HFVV) realizados en veintidós pacientes adultos con insuficiencia renal aguda, entre setiembre de 1991 y Julio de 1992.
