Background: Extensively hydrolyzed formulas are used for the dietary management of cow's milk allergy. There is a limited repertoire of commercially available formulas for infants with cow's milk allergy. Methods: This was a multi-center, double-blind, randomized, crossover clinical study of 75 children younger than 12 years, 11 months with documented cow's milk allergy. The primary outcome was the incidence of reactivity of a new extensively hydrolyzed casein formula and Nutramigen ® during a Double-Blind Oral Food Challenge. A subset of subjects under the age of three years, 11 months at the time of enrollment received either the new formula or Nutramigen ® for 16 weeks to investigate tolerance, taste preferences, growth and nutritional variables. Results: Sixty-one subjects completed both challenges in the per-protocol groups. The new formula was noninferior to Nutramigen regarding allergic symptoms of reactivity in both per protocol and intent to treat analyses, the latter addressing risk of bias from attrition during DBOFC. Both formulas met the American Academy of Pediatrics criteria for hypoallergenicity and had similar adverse event profiles. The new formula was comparable to Nutramigen ® in supporting growth and tolerance. Conclusion: The new hypoallergenic formula broadens access to formulations for dietary management of children with cow's milk allergy. Intent to treat analyses should be included in DBOFC studies of hypoallergenicity to reduce risk of bias from early discontinuation.

A Case of the Early Development of Atopic Dermatitis in a Nursing Infant

Background. The early (in the first months of life) formation of atopic status and the development of allergies are not uncommon pathological conditions in a pediatrician's practice, requiring sufficiently studied and theoretically grounded measures for the organization of primary/secondary prevention.Case Report. The parents visited a doctor with complaints of widespread skin rashes and troubled night sleep in a child from the age of two months. A burdened history of allergies of the child was traced through the female lineage (food allergy in the mother, maternal sister and grandmother). The perinatal period is complicated by acute respiratory infection in the third trimester and by maternal nutritional preferences (consumption of goat's and whole cow's milk). The delayed (on the 5th day of life) breastfeeding initiation, feeding with cow's milk-based formula, living next to an industrial enterprise, maternal choices of products with a high sensitizing potential not only during pregnancy but also during breastfeeding probably caused the early formation of atopic status — dry skin, widespread papular rash, microvesicles on the cheeks, hips, shins, scratching traces, serous-bloody crusts objectively defined at admission. The SCORAD scores corresponded to severe atopic dermatitis. The ImmunoCAP technology revealed high antibody titres to a number of products, including cow's milk and chicken egg proteins. Based on the findings, a therapeutic diet aimed at secondary prevention of allergic diseases, including respiratory allergies, was developed for the child.Conclusion. A case of the early (from 2 months) formation of atopic status with the development of atopic dermatitis caused by polyvalent food sensitization is described. A therapeutic diet containing products with a low sensitizing potential was prescribed for the child. The suggested therapy including the diet should prevent the progression of an allergic disease.

This review highlights research advances and important achievements in food allergy, anaphylaxis, and drug allergy that were published in the Journals of the European Academy of Allergy and Clinical Immunology (EAACI) in 2017. Food allergy and anaphylaxis research have continued to rapidly accelerate, with increasing numbers of outstanding developments in 2017. We saw new studies on the mechanisms, diagnosis, prevention of food allergy, and novel food allergens. Drug hypersensitivity, as well as hereditary angioedema, has been highlighted in the present review as the focus of recent developments. The EAACI owns three journals: Allergy, Pediatric Allergy and Immunology (PAI), and Clinical and Translational Allergy (CTA). One of the major goals of the EAACI is to support health promotion in which prevention of allergy and asthma plays a critical role and to disseminate the knowledge of allergy to all stakeholders including the EAACI junior members.
