В статье приведены сведения о причинах развития недостаточности витаминов, биологической ценности витаминов D, С, А. Освещены данные реальной клинической практики, позволяющие врачу принять решение о назначении комплекса витаминов для профилактики гиповитаминоза у детей первого года жизни.
The current development of medicine and the results of recent large-scale academic research in pediatrics provide the convincing conclusions that the formation of human health begins in the antenatal period of ontogenesis and continues throughout the infancy. The ideas and subsequent academic research on the influence of nutrition during the first thousand days of life on the programming of metabolism and the development of some chronic somatic diseases such as obesity, hypertension, coronary heart disease have become widespread. In addition, at the same period of life immune abnormalities with a predominance of one of the subpopulations, Th1 or Th2, can possibly form in children at risk of developing allergies when immune response is developing. The predominance of the Th2 cytokine profile (hyperproduction of interleukins 4, 5, 13, etc.) suggests the possibility of stable formation of the atopic status in a child afterwards. Consequently, the application of academic knowledge on the health status programming by nutrition during early ontogenesis is an important tool in preventive pediatric practice. Развитие современной медицины, результаты последних масштабных научных исследований в педиатрии приводят к убедительным выводам, что формирование здоровья человека начинается в антенатальном периоде онтогенеза и продолжается на протяжении раннего детского возраста. Широкое распространение получили идеи и последовавшие за ними научные исследования о влиянии питания первых тысячи дней жизни в программировании метаболизма и развитии некоторых хронических соматических болезней, таких как ожирение, гипертоническая болезнь, ишемическая болезнь сердца. Вместе с тем в этот же период жизни у детей групп риска по развитию аллергии при становлении иммунного ответа возможно программирование иммунных отклонений с преобладанием одной из субпопуляций -Th1 или Th2. Преобладание цитокинового профиля Th2 (гиперпродукция интерлейкинов 4,5, 13 и др.) предполагает возможность стойкого формирования атопического статуса ребенка в дальнейшем. Следовательно, использование научных знаний о программировании состояния здоровья питанием в процессе раннего онтогенеза является важным инструментом в практической профилактической педиатрии. The poll revealed a lack of correspondence between the parents' idea of vaccination and the paediatricians' attitudes towards vaccination calendar violations. Conclusion. Educational programs for doctors and parents covering the topic of vaccination can provide an effective resistance to the present anti-vaccination lobby. At the moment, the key issues are the necessity to decrease unjustified medical rejections for vaccinations, a continuous attention to the child's vaccination status (at any addressing) and informing the parents about the diseases which can be prevented through immunization.Key words : vaccination, anti-vaccination campaign, National Vaccination Calendar, children, vaccination, educational АКТУАЛЬНОСТЬВакцинопрофилактика имеет более чем 200-летнюю историю. Целесообразность и безопасность вакцинации обсуждается со времени первых опытов Э. Дженнера и Л. Пастера до наших дней. История разработки и при-менения вакцин содержит немало трагических фактов, связанных с ошибками производителей вакцин, неожи-данными проявлениями активности патогенных факто-ров, несовершенством методов инактивации и очистки препаратов. Background. The early (in the first months of life) formation of atopic status and the development of allergies are not uncommon pathological conditions in a pediatrician's practice, requiring sufficiently studied and theoretically grounded measures for the organization of primary/secondary prevention.Case Report. The parents visited a doctor with complaints of widespread skin rashes and troubled night sleep in a child from the age of two months. A burdened history of allergies of the child was traced through the female lineage (food allergy in the mother, maternal sister and grandmother). The perinatal period is complicated by acute respiratory infection in the third trimester and by maternal nutritional preferences (consumption of goat's and whole cow's milk). The delayed (on the 5th day of life) breastfeeding initiation, feeding with cow's milk-based formula, living next to an industrial enterprise, maternal choices of products with a high sensitizing potential not only during pregnancy but also during breastfeeding probably caused the early formation of atopic status — dry skin, widespread papular rash, microvesicles on the cheeks, hips, shins, scratching traces, serous-bloody crusts objectively defined at admission. The SCORAD scores corresponded to severe atopic dermatitis. The ImmunoCAP technology revealed high antibody titres to a number of products, including cow's milk and chicken egg proteins. Based on the findings, a therapeutic diet aimed at secondary prevention of allergic diseases, including respiratory allergies, was developed for the child.Conclusion. A case of the early (from 2 months) formation of atopic status with the development of atopic dermatitis caused by polyvalent food sensitization is described. A therapeutic diet containing products with a low sensitizing potential was prescribed for the child. The suggested therapy including the diet should prevent the progression of an allergic disease.
