Purpose Recombinant human erythropoietin (rHuEPO) is known to increase thrombotic risk in patients and might have similar effects in athletes abusing the drug. rHuEPO is prohibited by anti-doping legislation, but this risk has not been investigated thoroughly. This analysis was designed to evaluate whether rHuEPO impacts hemostatic profile and endothelial and platelet activation markers in trained subjects, and whether the combination with exercise affects exercise induced alterations. Methods This double-blind, randomized, placebo-controlled trial enrolled healthy, trained male cyclists aged 18-50 years. Participants were randomly allocated (1:1) to receive subcutaneous injections of rHuEPO (epoetin-β; mean dose 6000 IU per week) or placebo (0.9% NaCl) for 8 weeks. Subjects performed five maximal exercise tests and a road race, coagulation and endothelial/platelet markers were measured at rest and directly after each exercise effort. Results rHuEPO increased P-selectin (+ 7.8% (1.5-14.5), p = 0.02) and E-selectin (+ 8.6% (2.0-15.7), p = 0.01) levels at rest. Maximal exercise tests significantly influenced all measured coagulation and endothelial/platelet markers, and in the rHuEPO group maximal exercise tests led to 15.3% ((7.0-24.3%), p = 0.0004) higher E-selectin and 32.1% ((4.6-66.8%), p = 0.0207) higher Platelet factor 4 (PF4) levels compared to the placebo group. Conclusion In conclusion, rHuEPO treatment resulted in elevated E-and P-selectin levels in trained cyclists, indicating enhanced endothelial activation and/or platelet reactivity. Exercise itself induces hypercoagulability, and the combination of rHuEPO and exercise increased E-selectin and PF4 levels more than either intervention alone. Based on this, exercise potentially increases thrombotic risk, a risk that might be enhanced in combination with rHuEPO use.

Context:Crataegus aronia (Willd.) Bosc (Rosaceae) (syn. Azarolus L) is traditionally used to treat cardiovascular disorders.Objectives: To investigate C. aronia protection against a high-fat diet (HFD)-induced vascular inflammation in rats.Materials and methods: Wistar Male rats (180–220 g) were divided (n = 10/group) as control fed a standard diet (STD), STD + C. aronia (200 mg/kg, orally), HFD, HFD + C. aronia and HFD post-treated with C. aronia. Simvastatin (20 mg/kg) was co- or post-administered as a positive control drug. HFD was given for 8 weeks, and all other treatments were administered for 4 weeks.Results: Most significantly, co-administration of C. aronia to HFD-fed rats reduced the thickness of aorta tunica media (90 ± 5 vs. 160 ± 11.3 µm) and adventitia (54.3 ± 3.8 vs. 93.6 ± 9.4 µm). It also lowered protein levels of TNF-α (0.51 ± 0.15 and 0.15 ± 0.16 vs. 0.1 ± 0.09%) and IL-6 (0.52 ± 0.19 vs. 1.0 ± 0.2%) in their aorta or serum (5.9 ± 0.91 vs. 12.98 ± 1.3 ng/mL and 78.1 ± 6.7 vs. 439 ± 78 pg/mL, respectively). It also lowered all serum lipids and increased aorta levels of GSH levels (70.4 ± 4.0 vs. 40.7 µM) and activity of SOD (5.7 ± 0.7 vs. 2.9 ± 0.6 U/mg) and decreased serum levels of ox-LDL-c (566.7 ± 46 vs. 1817 ± 147 ng/mL). Such effects were more profound than all other treatments.Conclusions:C. aronia inhibits the HFD-induced vascular inflammation and its use in clinical trials is recommended.

The expression of microRNA-23b in peripheral blood leukocytes of patients with sepsis was investigated to assess the correlations with leukocyte, E-selectin, ICAM-1 and disease severity. The expression of microRNA-23b in peripheral blood leukocytes from 87 patients with sepsis, 50 patients with systemic inflammatory response syndrome (SIRS) and 50 normal controls were measured by reverse transcription-quantitative PCR (RT-qPCR), and stability of microRNA-23b was evaluated. Enzyme-linked immunosorbent assay (ELISA) was used to detect E-selectin and ICAM-1. Sequential organ failure assessment (SOFA) scoring system was used to assess the severity of sepsis patients. Correlation analysis was performed between microRNA-23b and the total number of white blood cells (WBC), E-selectin, ICAM-1, and SOFA scores. Compared with the normal control group, the expression level of microRNA-23b in the sepsis group was significantly decreased (P<0.05), and WBC and E-selectin were significantly increased (P<0.05). ICAM-1 level in the sepsis and the SIRS groups was significantly higher than that in the control group (P<0.05), and it is also higher in the sepsis group than in the SIRS group. In the sepsis group, the expression level of microRNA-23b in the death group was significantly lower than that in the survivor group (P<0.05), while the level of E-selectin, ICAM-1, and SOFA scores were significantly higher in the death group than in the survivor group (P<0.05), while there was no significant difference in WBC between the groups (P>0.05). The expression level of microRNA-23b in patients with sepsis was significantly negatively correlated with SOFA scores, E-selectin, and ICAM-1 (r=−0.633, −0.585, and −0.439, respectively, P<0.05). The expression of microRNA-23b in peripheral blood of patients with sepsis is related to the manifestation of the inflammatory state, and can be used to judge the severity and prognosis of patients with this disease.
