Background The data on the epidemiology of epilepsy are limited for developing countries including India. We estimated the incidence of epilepsy in a cohort of service personnel from India followed for over two decades. Materials and Methods The data for this epidemiological study were derived from the electronic medical records (EMRs) of the male service personnel. The participants (age < 18 years) were enrolled into active service between 1990 and 2015 in good health. The data pertaining to the diagnosis of epilepsy were derived from the EMR using the prevalent International Classification of Diseases codes. We calculated the incidence rate as per person-years (py) using appropriate statistical methods. Results Our data included 51,217 participants (median age: 33 years, range: 17–54) with a mean follow-up of 12.5 years, giving a cumulative follow-up duration of 613,925 py. A total of 291 patients developed epilepsy during the study, giving an incidence rate of 0.47 per 1000 py (95% confidence interval: 0.42–0.53). Undifferentiated spondyloarthropathy, central nervous system disorders, and alcohol dependence syndrome were the common comorbid ailments in patients with epilepsy. Conclusion Our cohort had a comparable incidence rate of epilepsy with other studies from India and abroad.

Остеопороз и остеопатии / Osteoporosis and Bone Diseases | 108 CASE REPORT АКТУАЛЬНОСТЬПервичный гиперпаратиреоз (ПГПТ) -первичная, т. е. не спровоцированная пониженным уровнем каль-ция в крови, гиперпродукция паратгормона (ПТГ), кото-рая приводит к нарушению гомеостаза кальция.Заболеваемость ПГПТ составляет около 20 случаев на 100 тыс. населения в год; в течение последних лет на-блюдается ее неуклонный рост в связи с повышением и информативности и доступности методов диагностики ПГПТ. Распространенность ПГПТ в популяции составля-ет около 1%, увеличивается с возрастом и достигает 2% Сложности в топической диагностике первичного гиперпартиреоза в ряде случае обусловлены вариабельностью расположения околощитовидных желез. Наиболее часто верхние околощитовидные железы обнаруживаются по за-днемедиальной поверхности правой и левой долей щитовидной железы (верхняя треть доли), хотя встречаются и на уровне бифуркации общей сонной артерии, за глоткой и пищеводом, и интратиреоидно (1%). Расположение нижних околощитовидных желез более вариабельно: по боковой или задней поверхности, или ниже нижнего полюса щитовидной железы на расстоянии 0,2-1,5 см, в рогах тимуса, в переднем или заднем средостении, интратиреоид-но (1-2%). Это объясняется сложным и продолжительным характером миграции зачатка из III-IV жаберного кармана в ходе эмбриогенеза. Топическая диагностика при первичном гиперпаратиреозе опирается на данные УЗИ органов шеи, сцинтиграфии с технетрилом, мультиспиральной компьтерной томографии органов шеи и средостения. При со-четании многоузлового зоба и интратиреоидного расположения околощитовидной железы могут возникнуть допол-нительные сложности в верификации различных узлов (узлы щитовидной железы или околощитовидные железы). В настоящей статье обсуждается два клинических случая интратиреоидного расположения околощитовидных желез. Показан алгоритм топического подтверждения диагноза. Определение уровня паратгормона в пунктате из подозри-тельных образований, которые могут представлять собой интратиреоидные околощитовидные железы или узлы щи-товидной железы, поможет избежать диагностических ошибок. In this article, we discuss difficulties in parathyroid localization modalities in diagnosis of primary hyperparathyroidism. Most often, superior parathyroid glands are located on the posteromedial surface of the right and left lobes of the thyroid gland, however, they also could be found at the carotid bifurcation, behind pharynx and esophagus, as well as inside thyroid gland. Location of the inferior parathyroid glands is more variable: on the side or back surface, or below the lower pole of the thyroid gland, as well as in thymus, posterior or anterior mediastinum, or inside thyroid. Localization modalities of primary hyperparathyroidism are based on neck ultrasonography, scintigraphy with sestamibi, computed tomography of neck and mediastinum. Chronic hypoparathyroidism is characterized by low serum calcium, increased serum phosphorus, and inappropriately low or decreased serum parathyroid hormone. This rare disorder is associated with a variety of complications. The prevalence, incidence, mortality, financial burden, and epidemiology of complications of this disorder are not well understood. This narrative review summarizes current information on the epidemiology and complications of chronic hypoparathyroidism. The reported prevalence of chronic hypoparathyroidism ranges from 6.4–37/100,000, and the incidence is reported to be 0.8–2.3/100,000/year. Mortality is not increased in studies from Denmark or South Korea but was increased in studies from Scotland and Sweden. The financial burden of this disorder is substantial because of increased health care resource utilization in two studies but not well quantitated. Recognized complications include hypercalciuria, nephrocalcinosis, kidney stones, and chronic kidney disease; low bone turnover and possibly upper extremity fractures; cardiac and vascular calcifications; basal ganglia calcifications, cataracts, infections, neuropsychiatric complications, and difficulties with pregnancy. This review concludes that chronic hypoparathyroidism is a rare disorder associated with significant morbidity that may not increase overall mortality but is associated with a substantial financial burden. © 2022 The Authors.
