Previous population-based studies reported the incidence and mortality of ischemic stroke (IS) subtypes in different settings, most of them on the White population from high-income countries 1,2,3,4,5,6,7,8 . In Latin America, there is only one study, which was conducted in a Spanish and Mestizo population in Iquique, Chile 9. As far as we know, no previous study has reported IS subtypes rates among predominantly White people from Latin America.The crude annual incidence rate of IS in Brazil range from 62 to 92 per 100,000 inhabitants with a mixed pattern of cardiovascular risk, characterized by high prevalence of ischemic heart disease, dyslipidemia, hypertension and diabetes 10,11 . Determining the incidence, mortality and risk factors among IS subtypes may help to improve the public health actions and reduce the IS disease burden 12,13 . ABstrAct Aims: To measure the incidence and mortality rates of ischemic stroke (IS) subtypes in Joinville, Brazil. Methods: All first-ever IS patients that occurred in Joinville from January 2005 to December 2006 were identified. The IS subtypes were classified by the TOAST criteria, and the patients were followed-up for one year after IS onset. Results: The age-adjusted incidence per 100,000 inhabitants was 26 (17-39) for large-artery atherosclerosis (LAA), 17 (11-27) for cardioembolic (CE), 29 (20-41) for small vessel occlusion (SVO), 2 (0.6-7) for stroke of other determined etiology (OTH) and 30 (20-43) for stroke of undetermined etiology (UND). The 1-year mortality rate per 100,000 inhabitants was 5 (2-11) for LAA, 6 (3-13) for CE, 1 (0.1-6) for SVO, 0.2 (0-0.9) for OTH and 9 (4-17) for UND. Conclusion: In the population of Joinville, the incidences of IS subtypes were similar to those found in other populations. These findings highlight the importance of better detection and control of atherosclerotic risk factors.

Among ischemic strokes, cardioembolic (CE) stroke has the worst outcome. We measured the incidence of atrial fibrillation (AF) and non-AF related CE strokes, previous anticoagulant use, and the 90-day functional outcome. Using multiple overlapping sources, we included all CE strokes that occurred in 2015 in Joinville, Brazil. Of the 374 ischemic strokes, 23% (84) were CE. The CE age-adjusted incidence, per 1,000 person-years, was 0.14 (0.11-0.17). Of the 26 patients with known prior AF, 73% (19) were not anticoagulated, 77% (20) had a CHA 2 DS 2 VASc score ≥ 3 and 81% (21) had a HAS-BLED score < 3. After three months, approximately one third of those 26 patients died or became disabled. The incidence of CE stroke in our sample was lower than in other population-based studies. The opportunity for anticoagulation was missed in one third of cases.Keywords: stroke; incidence; prognosis; population studies in public health. RESUMOEntre todos os subtipos de AVC isquêmico, os eventos cardioembólicos (CE) apresentam os piores prognósticos. Determinamos a incidência de AVC isquêmico CE, associada ou não à fibrilação atrial (FA), o uso prévio de anticoagulantes e os desfechos funcionais em Joinville, Brasil. Utilizando múltiplas e sobrepostas fontes de informação, registramos todos os primeiros eventos CE ocorridos em 2015. Entre 374 eventos isquêmicos, 23% (84) foram CE. A incidência ajustada, para qualquer fonte cardioembólica, foi 0.14 (0.11-0.17) por 1000 pessoas-ano. Entre 26 pacientes com FA previamente conhecida, 73% (19) não estavam anticoagulados, 77% (20) tinham ≥ 3 pontos na escala CHA 2 DS 2 VASc e 81 % (21) < 3 pontos na escala HAS-BLED. Após 3 meses, um terço destes 26 pacientes morreram ou ficaram dependentes. A incidência de AVC I CE em Joinville é menor do que em outros estudos de base populacional. Um terço dos casos de AVC I CE com FA previamente conhecida perderam a oportunidade de anticoagulação.Palavras-chave: acidente cerebral vascular; incidência; prognóstico; estudos populacionais em saúde pública.Worldwide, ischemic stroke is the most common major pathological type of stroke 1 . Among all ischemic stroke subtypes, cardioembolic (CE) stroke is particularly worrisome for two main reasons. The first one is demographic: as age is a risk factor for both atrial fibrillation (AF) and stroke, the aging of the population will progressively increase the burden of CE stroke in many countries.1,2 The second reason involves outcomes: in a population-based study in Oxfordshire, UK, 2,555 ischemic stroke subtypes were followed up from 2002 to 2014. After six months, two thirds were dependent. After one year, 42% had died. These outcomes are significantly higher than those reported for other ischemic stroke subtypes 2 . Therefore, knowing the incidence of CE stroke is crucial for primary and secondary prevention policies 1,2,3 .

SažetakUvod: Moždani je udar među vodećim uzrocima smrtnosti i invalidnosti u svijetu, a prvi je uzrok invalidnosti u Republici Hrvatskoj. Kao posljedica moždanog udara nastaje različiti stupanj tjelesnog, kognitivnog i psihosocijalnog poremećaja. Bolesnicima je najteža posljedica nakon moždanog udara gubitak sposobnosti obavljanja aktivnosti svakodnevnog života i ovisnost o tuđoj pomoći. Oporavak bolesnika ovisi o vrsti moždanog udara, mjestu zahvaćenosti te težini onesposobljenosti, potpori obitelji i održavanju postojećeg stanja. Primarni je cilj neurorehabilitacije funkcionalni napredak kroz maksimalno moguće vraćanje neovisnosti životnog funkcioniranja. Za što uspješniju rehabilitaciju nužno je aktivno sudjelovanje obitelji bolesnika, specifična edukacija osoblja te rani početak rehabilitacije.Cilj: Cilj provedenog istraživanja bio je ispitati učinko-vitost Bobath tretmana i klasičnog fizioterapijskog tretmana na obavljanje aktivnosti svakodnevnog života kod bolesnika nakon moždanog udara. Materijali i metode:U istraživanje je uključeno 30 ispitanika koji su podijeljeni unutar dvije eksperimentalne skupine. Osnovni kriterij za odabir ispitanika bila je prisutnost moždanog udara. Prva eksperimentalna skupina E1 tretirana je klasičnim fizioterapijskim tretmanom, dok je druga eksperimentalna skupina E2 tretirana Bobath tretmanom. Obje skupine bile su tretirane kroz 15 dana, unutar tri tjedna, a tretman je trajao 45 minuta dnevno. Procjena aktivnosti svakodnevnog života mjerena je standardiziranim Barthelovim indeksom, a provedena je na početku i na kraju tretmana.Rezultati: Istraživanje je pokazalo kako učinci klasičnog fizioterapijskog tretmana i Bobath tretmana nisu statistički značajni. Prema dobivenim rezultatima Barthel indeksa vidljivo je da Bobath tretman ima veći učinak na poboljšanje obavljanja aktivnosti svakodnevnog života kod bolesnika nakon moždanog udara za razliku od klasičnog fizioterapijskog tretmana, no statistički to nije potvrđeno. Zaključak:Rezultati između inicijalnih i završnih mjerenja u obje skupine ispitanika nisu pokazali statistički značajne učinke u obavljanju aktivnosti svakodnevnog života, iako rezultati ukazuju na pozitivnu tendenciju obje skupine u samostalnosti obavljanja aktivnosti svakodnevnog života, posebno u drugoj skupini ispitanika.
