Incidence and risk of advanced age-related macular degeneration in eyes with drusenoid pigment epithelial detachment

Abstract: To investigate the incidence and risk of advanced age-related macular degeneration (AMD), including geographic atrophy (GA) and macular neovascularization (MNV), in eyes with drusenoid pigment epithelial detachment (PED). Eighty-five eyes with drusenoid PED from 85 patients (77.2 ± 7.0 years, male/female: 44/41) were included in this study. Patients were followed up every 1–3 months via spectral-domain optical coherence tomography (SD-OCT) and color fundus photography. If exudation was observed on SD-OCT, fluo… Show more

“…Drusenoid PED pathogenesis and its implication in nAMD is still unknown but it is though that they are associated with an increased risk of advanced AMD with atrophy or brosis development (34). Thus, achieving a attening or reduction in a PED early in disease might be of bene t. Javaheri et al demonstrated that one-third of 586 eyes had a attened PED after an initial ranibizumab injection (35).…”

“…Shijo et al 18 studied the 5‐year cumulative incidence of advanced AMD in eyes with dPEDs, including GA and nAMD, being ~30%. A larger baseline dPED width was the only risk factor detected for advanced AMD.…”

“…Drusenoid pigment epithelial detachments (dPED) are characterised by the confluence of soft drusen, generally in the centre of the macula, containing a variable amount of serous and drusenoid material on OCT and with variable amount of pigmentary changes associated (Figure 2, panels A,B) [18][19][20] Cukras et al defined these lesions as 350 μm in diameter or greater. Shijo et al 18 studied the 5-year cumulative incidence of advanced AMD in eyes with dPEDs, including GA and nAMD, being 30%.…”

“…Drusenoid PED was defined as an elevated mound with confluent soft drusen .500 mm on spectraldomain OCT (SD-OCT) as in previous investigations. 13,14 Only cases followed up for .24 months after the initial diagnosis were included for the analysis. If both eyes showed drusenoid PED, only the right eye was included.…”
