The first fluoride varnishes were developed during the 1960s (Duraphat^® sodium fluoride varnish) and 1970s (Fluor Protector^® silane fluoride varnish) to prolong the contact time between fluoride and enamel. Fluoride varnishes adhere to enamel, and calcium fluoride formed after application acts as a long-term reservoir of fluoride. Currently Duraphat varnish is the most widely used topical fluoride for professional application in Europe, and the use of fluoride varnishes is increasing in the USA. Duraphat varnish has been effective in three decades of clinical studies, but the results of Fluor Protector varnish are inconclusive. The percent caries reductions found in the 1990s have generally been lower than those reported in earlier studies, probably because of the higher exposure to other preventive measures in the more recent studies. In studies comparing Duraphat varnish and APF gel, Duraphat varnish was equally or more effective than APF gel. Sealants were most effective in preventing occlusal caries. Four applications per year, or three weekly applications once a year, have been found to be effective. However, several studies have shown that two applications per year may provide comparable results. Application is fast and easy. Professional prophylaxis is not necessary, and the patient can leave immediately after the treatment. No acute toxicity has been reported after using any fluoride varnish.

The aim of this in situ investigation was to study the effect of an amine fluoride/sodium fluoride mouthrinse (total F = 250 ppm) in addition to an amine fluoride dentifrice (F = 1,250 ppm) on the amount of acquired fluoride in enamel and dentine. In the partial prosthesis of 12 participants a combined specimen of slightly demineralized enamel and dentine was placed. During two consecutive experiments, each lasting 3 weeks, the participants used an amine fluoride dentifrice alone or in combination with a fluoride mouthrinse. After the in situ period the specimens were retrieved and both the enamel and the dentine specimens were analysed for the amount of KOH–soluble fluoride and structurally bound fluoride. The results showed a significant increase in both KOH–soluble and structurally bound fluoride in enamel and dentine when a fluoride mouthrinse was used. Whether the rinsing procedure was performed immediately after toothbrushing or with a delay of 2 h did not influence the results. Furthermore it was shown that dentine acquired substantially more fluoride than enamel during the experimental period. The results indicate that a fluoride mouthrinse used in addition to a fluoride dentifrice may have a beneficial effect on the protection of enamel and dentine against caries.

The aim of this in vitro study was to examine whether the concentration of amine fluoride solution influences the uptake of fluoride by enamel after topical treatment. Four slabs were prepared from each of 31 impacted third molars and selected at random so that one slab from each tooth was present in each group. One group of slabs served as an untreated control (D). The slabs of the other three groups were shaken for 3 min in an amine fluoride solution on 3 successive days: group A (1% F), group B (0.5% F) and group C (0.25% F). The amounts of KOH-soluble fluoride produced with 1 and 0.5% amine fluoride solution did not differ significantly and were superior to 0.25% amine fluoride solution. A statistically significant increase in structurally bound fluoride was observed in groups A, B and C compared with control group D. The difference in the amounts of structurally bound fluoride was significant only between 1 and 0.25% F concentrations. These data suggest that the fluoride content in an amine fluoride solution (1%) can be decreased by half without reducing its ability to form alkali-soluble or structurally bound fluoride. The results of this study are consistent with those of previous clinical studies.
