This paper presents a continuous improvement (CI) project based on the application of Lean Six Sigma (LSS) methodology. This project was carried out at coffee manufacturing industry located in northern area of Malaysia. The focus of this presented CI project is to improve the weight inconsistency problem of coffee powder packaging. The presented CI project was mainly guided with Six Sigma methodology of Define, Measure, Analyze, Improve and Control (DMAIC) steps, and related Lean tools applications. The step of Define summaries the overall view of CI project. In the step of Measure, current level of sigma based on defect per million of opportunities (DPMO) calculation was determined. Initially, 6% of rejected products were recorded, which it presents the sigma level at 3.1. In the step of Analyze, a series of team-based activities towards root cause identification was carried out. It included the applications of cause effect (CE) analysis and possible causes prioritization and close observations on packaging process. In the step of Improve, the related Lean tool was proposed to improve the problem under study. In the final step of Control, related supportive actions were suggested to sustain the effectiveness of the proposed solution. There are fourteen possible causes are initially identified in CE analysis and based on team’s evaluation process of possible causes prioritization four causes are collectively to be the most possible of root cause(s). Three close observation sessions were carried out to finalize the most possible root cause of the problem. Result strongly suggested that the high variation of coffee powder’s size and inconsistency is the root cause. An improvement strategy based on Lean Manufacturing approach called ‘poka yoke’ is proposed. The proposed improvement strategy was then validated based on real packaging process scenario. Result shows that the proposed improvement strategy is significantly effective to solve the problem with 0% of reject product was recorded so far, where it is not only produced the product within the acceptable weight of 215 and 208 grams, but also presents the reduction of weight fluctuation that close to ideal weight of 208 grams.

PG Madukismo sebagai salah satu pabrik gula di Indonesia dalam proses produksinya masih ditemukan sebagian produk gula yang cacat. Produk cacat tersebut merugikan perusahaan karena menyebabkan pemborosan biaya dan waktu produksi. Produk gula cacat yang diolah kembali dapat mengurangi komposisinya terutama zat-zat yang bermanfaat untuk dikonsumsi. Penelitian ini bertujuan untuk memberikan usulan perbaikan pengendalian kualitas untuk meningkatkan kualitas produksi gula dan meminimalisasi terjadinya produk cacat. Metode yang digunakan dalam penelitian ini adalah metode six sigma yang terdiri dari lima tahap yaitu tahap define, measure, analyze, improve, dan control. Tahap define bertujuan mengidentifikasi alur proses produksi. Tahap measure dilakukan perhitungan tingkat pengendalian produk cacat. Tahap analyze bertujuan menganalisis penyebab produk cacat. Tahap improve menggunakan metode FMEA untuk mengidentifikasi mode kegagalan penyebab produk cacat dan usulan perbaikan. Tahap control bertujuan mengimplementasikan tindakan perbaikan dan melakukan pengawasan proses produksi. Hasil penelitian menunjukkan bahwa mode kegagalan kualitas tebu tidak sesuai standar memiliki nilai RPN paling besar yaitu 168. Tindakan yang disarankan untuk memperbaiki pengendalian kualitas produksi gula adalah meningkatkan pengecekan kualitas tebu, meningkatkan ketelitian dalam pembersihan tebu sebelum digiling dan menentukan jumlah batas maksimal proporsi cacat per hari.

Product quality has now become one of the main factors that every company must have to compete globally. One way is to reduce the number of defective products in the production process. This method is a continuous improvement effort made by PT Toyota Motor Manufacturing Indonesia and companies engaged in the automotive sector that produces cars. One of the processes is packing roof panels whose output will be sent to importing countries where the number of requests per month is relatively high but still has a high defect rate. Based on production data and defect data for the January-February 2019 period, roof panel packaging production has a 1% -3% defect rate. This company must reduce the defect rate to achieve the company target of zero defects. This study aims to improve the quality of the packaging process by minimizing the number of defective products. This study uses the Six Sigma method with DMAIC phases. This method has several phases, namely, Define, Measure, Analysis, Improvement, and Control. Corrective action to reduce product defects based on 5W + 1H is to formulate a repair plan at the upgrade phase, namely by making SOPs spraying anti-rust and supervising operators who produce roof panel packaging. The improvement results reduced the DPMO value from 33,500 units to 2,050 units and increased the sigma level from 3.33 to 4.37. The DMAIC phase effectively controls and improves product quality levels in the automotive industry.
