This study was an examination of the refractive results and complications encountered with implantation of the Eyecryl phakic intraocular lens (IOL) (Biotech Vision Care, Luzern, Switzerland). Methods: IOL recipients with a follow-up period of at least 2 years were included the study. Uncorrected and corrected distance visual acuity (UDVA and CDVA, respectively), central endothelial cell density (ECD), and procedure-related complication data were recorded. Results: The study included 52 eyes from 26 patients. The efficacy index (postoperative UDVA/preoperative CDVA) was 1.32±1.09 in the first year and 1.39±1.03 in the second year. The safety index (postoperative CDVA/preoperative CDVA) was 1.69±1.16 at the last follow-up. None of the patients lost 2 or more lines of CDVA. The mean ECD loss was-4.51±9.69% at 1 year (p=0.005). There was no statistically significant ECD loss between the 1-and 2-year follow-up visits (p=0.794). Conclusion: The results of Eyecryl phakic IOL implantation were favorable in terms of efficacy and safety.

Comparative study of anterior segment measurements using 3 different instruments in myopic patients after ICL implantation

Background To compare and correlate anterior segment measurements of myopic eyes implanted with Implantable Collamer lens (ICL V4c) by using anterior segment optical coherence tomography (AS-OCT), Pentacam and ultrasound biomicroscopy (UBM). Methods Anterior chamber depth (ACD), distance between corneal endothelium and anterior surface of ICL(C-ICL) and central vault were measured in 82 phakic myopic eyes of 82 patients who underwent ICL surgery, by using AS-OCT, Pentacam and UBM consecutively at 3 months follow up. The correlation and agreement of instruments were accessed by using Intraclass correlation coefficient (ICC) and the Bland-Altman plot. Results AS-OCT showed higher ACD, C -ICL and central vault measurements than both of Pentacam and UBM ( P < 0.001), while Pentacam showed lower measurements than UBM ( P < 0.05). The Pearson correlation coefficient (r) was 0.91 to 0.96, and ICC was 0.95 to 0.98 for all measurements between difference devices (all P < 0.001). The 95% limits of agreement of ACD, C-ICL, vault measurements were 0.13 to 0.38 mm, − 0.07 to 0.27 mm, 0.08 to 0.34 mm between AS-OCT and Pentacam, − 0.03 to 0.33 mm, − 0.16 to 0.31 mm, − 0.10 to 0.26 mm between AS-OCT and UBM, and − 0.29 to 0.07 mm, − 0.25 to 0.20 mm, − 0.31 to 0.05 mm between Pentacam and UBM, respectively. Conclusions AS-OCT demonstrated significantly higher value, while Pentacam demonstrated significantly lower value than UBM for ACD, C-ICL and central vault measurements in myopic eyes after ICL surgery. Measurements with these instruments were highly correlated, but could not replace each other especially for vault. This study provided valuable information about how to judge the results of anterior segment parameters of eyes implanted with ICL V4c from different devices. posterior chamber phakic intraocular lens (pioL) implantation is a common option for correcting moderate-to-high ocular refractive defects. Because this pIOL is implanted on ciliary sulcus, the distance between the back surface of the pIOL and the anterior surface of the crystalline lens, that it is known as vault, should be measured in different conditions to ensure the technique's safety. Cyclopentolate is a drug that dilates the pupil and relaxes accommodation (cycloplegia). It is often used for different ocular examinations and for other medical purposes. However, there is no evidence of the effect of this drug on vault. This study quantified central vault changes associated with cyclopentolate instillation. We measured the vault under normal conditions (pre-cycloplegic instillation) and after instilling cyclopentolate on 39 eyes of 39 patients with implanted pIOL. Our results suggest that cyclopentolate instillation may induce changes to vault in eyes with implanted pIOL. These changes seem safe and are mainly associated with vault under normal conditions, but also with anterior chamber depth, pupillary diameter and pIOL size. Visian Implantable Collamer Lens (ICL, STAAR Surgical) is a foldable posterior chamber phakic intraocular lens (pIOL) designed to correct moderate-to-high myopia 1. The latest models have a central hole that allows better aqueous humour exchange between the anterior and posterior chambers without the need for iridotomy 2. These models present safe and efficacious results as previous models with no hole 3,4. The central vault is defined as the distance between the back surface of the ICL and the anterior surface of the crystalline lens. An appropriate vault has been suggested to be between 250 and 750 µm 5. An insufficient vault may induce anterior subcapsular cataract due to the contact between the ICL and the anterior lens capsule 1,6,7. Excessive vaulting can cause angle closure, pupillary blockage or pigmentary dispersion syndrome, which can increase intraocular pressure and, thus, the risk of glaucoma 8-11. Therefore, it is important to measure this distance to ensure the technique's safety. Changes in anterior segment structures appear during accommodation, such as reduced anterior chamber depth (ACD), inducing proximity between the anterior surface of the crystalline lens and the cornea with appearance of synkinetic myosis 12. The ICL is implanted in the ciliary sulcus. Given its location, dynamic interactions take place between the pIOL and the surrounding structures. Some authors have studied these effects on the vault by accommodative stimulus 13-17 , instillation of pilocarpine for pharmacologically-induced accommodation 13,18 , light-induced pupil constriction 15,17,19 and mydriasis. 20 However, no studies have quantified the cycloplegic effect on the central vault.
