Impact of total variation regularized expectation maximization reconstruction on the image quality of 68Ga-PSMA PET: a phantom and patient study

Abstract: Objectives: To investigate the impact of total variation regularized expectation maximization (TVREM) reconstruction on the image quality of 68Ga-PSMA-11 PET/CT using phantom and patient data. Methods: Images of a phantom with small hot sphere inserts and 20 prostate cancer patients were acquired with a digital PET/CT using list-mode and reconstructed with ordered subset expectation maximization (OSEM) and TVREM with 7 penalisation factors between 0.01 and 0.42 for 2 and 3 minutes-per-bed (m/b) acquisition. “…In our study, the SUV of lesions in HYPER Iterative reconstructions was higher than that in OSEM reconstruction. The results were consistent with the findings of a previous study using 68 Ga-PSMA PET in which a significant increase in lesion SUV max was reconstructed with HYPER Iterative [ 18 ]. ΔSUV peak was correlated with lesion size which suggests that small lesions have larger SUV peak elevations than large lesions.…”

“…The image quality reconstructed using OSEM may not be able to determine the clinical diagnosis, and there is a necessary to find an effective reconstruction algorithm to improve the image quality to obtain qualified images. A new Bayesian penalized likelihood reconstruction algorithm (HYPER Iterative) developed by United Imaging compensates for this shortcoming [ 18 ]. The HYPER Iterative algorithm incorporates noise control into each iteration, and finds the maximum likelihood solution through repeated iterations.…”

“…Total variation regularized expectation maximization (TVREM), a new BPL algorithm, was introduced recently (HYPER Iterative, United Imaging Healthcare). Related studies have shown that TVREM algorithm in 68 Ga-PSMA imaging can ensure image quality while shortening the acquisition time ( 11 ). Therefore, as a BPL algorithm, TVREM has the potential to further increase the detection rate of lesions, shorten the acquisition time or reduce the dose (reduce administrated dose) for 68 Ga-DOTA-TATE PET imaging.…”

“…On the other hand, a longer acquisition time may adversely degrade the image quality due to the patient's motion during the acquisition. An alternative solution is to adapt the regularised images with less noise and better contrast, such as block sequential regularised expectation maximization (BSREM) [17,[29][30][31][32]. Moreover, the employment of the TB-PET images can make an unprecedented breakthrough on the image quality of the training dataset.…”
