IgA‐Associated Glomerulonephritis

Abstract: Mesangial IgA depostiiton was detected by routine fluorescent microscopy of 19 cases of glomerulonephritis. Twelve patients had a variable degree of diffuse mesangial proliferative glomerulonephritis. The heterogeneous histological findings in the other seven patients show that mesangial IgA deposition alone does not have diagnostic specificity. However, when interpretation of the mesangial IgA deposition is complemented by clinical and histological information, a group of patients with the characteristic c… Show more

“…More fulminant mucosal antigenic exposure with penetration of antigen into the extracellular fluid could lead to an associated IgG or IgM antibody response, which would favor C3 deposition. This idea is further supported by the observations of some authors that codeposition of IgG and/or IgM in humans correlates with a more severe disease (4)(5)(6)(7)12). Finally, species differences in the capacity of pure IgA immune complexes to promote full complement fixation may exist between mice and men.…”

“…Since IgA is the only con stant immunoglobulin, the term IgA mesangial nephrop athy is now preferred. An IgA-IgG combination was found in approximately 50% of the cases [10,22,24,25], IgA alone was found in 45.7% of 239 cases, accompanied by IgG in 50.1 %, IgM in 21.4%, C3 in 82.4%, and fibrin ogen in 37.2% of the patients [10]. Most workers have failed to find IgA secretory component (SC) in the glo merular mesangium [10,17,19,26,27].…”

“…Occasionally it may be the only immunoglobulin present (Berger, 1969;Hyman et al, 1973;Lowance et al, 1973;Roy et al, 1973;McCoy et al, 1974;Morel-Maroger and Verroust, 1975;Woodroffe et al, 1975;Zimmerman and Burkholder, 1975). We therefore reviewed our renal biopsy material and studied further those cases in which there were significant glomerular IgA deposits.…”
