Hypotension and pruritus induced by neuraxial anaesthesia in a cat

Abstract: Although preventive epidural morphine administration with bupivacaine is effective in producing long-lasting analgesia, neuraxial anaesthesia can cause cardiovascular depression and pruritus. This report presents the development and treatment of hypotension and pruritus after intrathecal morphine and bupivacaine administration in a 3-year-old female spayed Domestic Short-hair cat presented for surgical repair of a torn right cranial cruciate ligament. Opioid-induced pruritus is not usually considered a frequen… Show more

“…RM-ANOVA revealed a significant effect of propofol and group × time interaction (F[ (2,15] = 46.87, P < 0.001; F [22, 165] = 2.37, P = 0.001); compared with intralipid and normal saline groups, the scratching behavior was significantly attenuated in the propofol group (P < 0.001, Dunnett test) (Fig. 2).…”

“…Mild respiratory depression has been reported after epidural administration of 0.1 mg kg −1 of morphine in dogs (6) but this is not clinically relevant (38) if assisted or mechanical ventilation is promptly instituted. Pruritus has also been reported following epidural and intrathecal morphine administration (99101). The use of a 5-hydroxytryptamine antagonists (e.g., ondansetron) has been described as treatment for neuraxial opioid-induced pruritus (102).…”

“…To the authors’ knowledge, this is the largest study to date investigating safety and complications in cats receiving epidurals. Case reports exist in the veterinary literature of complications related to lumbosacral epidurals including hypotension, pruritis, chronic urinary and bowel dysfunction, and urinary retention 42‐46 . Because of concerns that urinary retention can occur, cats in this study were required to be hospitalized with an indwelling urinary catheter.…”
