Homicides and socio-environmental determinants of health in Brazil: a systematic literature review

Brazil currently has the highest absolute number of homicides in the world, which results from a complex range of factors. This study aimed at understanding the associations between socio-environmental determinants of health (SDH) and homicides in Brazil through a systematic literature review. The review followed PRISMA guidelines, selecting quantitative and qualitative studies published in Portuguese, English, and Spanish carried out between 2002 and 2017, available in the PubMed, MEDLINE, LILACS, SciELO and BVS-BIREME databases. Two trilingual reviewers tracked studies independently by basing on the eligibility criteria. We critically assessed the selected studies with the Critical Appraisal Skills Programme (CASP) or the Checklist for Analytical Cross-Sectional Studies, depending on the study design. We considered 60 studies and grouped their SDH into categories to develop a narrative synthesis about each SDH. These categories were: territory; race/ethnicity; gender; age; social inequalities and economic factors; development; education; work and employment; drugs and trafficking; other SDH. We found some SDH were more associated with homicides, such as being young, black, male, of low education level, and also people who lived in places of high social inequality, such as urban suburbs and agricultural frontiers. Unemployment and drug trafficking, as well as intersections between various SDH were also prominent. Education seems to be a protective factor for homicide. Despite the limited capacity of interpretation due to the high range of methodological approaches, this review shows the importance of considering SDH and their intersections when developing homicide prevention policies.

Mortalidade de homens jovens por agressões no Brasil, 2010-2014: estudo ecológico

O objetivo foi analisar a associação entre o risco de morte por agressões em jovens do sexo masculino e características sociodemográficas dos municípios brasileiros. Estudo ecológico tendo como unidades de análise os 1.651 municípios com mais de 20.000 habitantes. Foram utilizados dados do Sistema de Informação sobre Mortalidade (SIM) e indicadores obtidos do Censo Demográfico 2010 e do Atlas do Desenvolvimento Humano no Brasil. Razões de taxas de mortalidade foram estimadas por modelo de regressão binomial negativa. No período de 2010-2014, foram registrados 127.137 óbitos por agressão a jovens de 15-29 anos de idade do sexo masculino. A taxa corrigida de mortalidade foi 133,3/100 mil habitantes no conjunto dos municípios (mediana 71,5/100 mil habitantes). A taxa foi maior à medida que aumentou o porte populacional dos municípios. Razões de taxas mais elevadas no modelo ajustado foram observadas nos municípios mais urbanizados (1,95; IC95%: 1,70-2,23), em categorias intermediárias de desigualdade de renda (1,10; IC95%: 1,01-1,20) e proporção de pobreza (1,69; IC95%: 1,51-1,89), com menor proporção de jovens frequentando o Ensino Médio (2,05; IC95% 1,83-2,30), maior proporção de jovens de 18-24 anos desocupados (1,27; IC95%: 1,16-1,40) e maior número de mulheres em relação ao de homens (1,28; IC95% 1,05-1,58). A mortalidade de jovens do sexo masculino por agressão foi elevada, especialmente nos municípios maiores, mais urbanizados e com maior proporção de jovens buscando emprego e fora do ensino médio. Evidencia-se a relevância das políticas sociais para o enfrentamento da violência entre jovens.

Introduction: In Brazil, violence, regardless of the type, is the leading cause of death in adolescents and young adults.Objective: To describe the characteristics of the homicides in which female children and adolescents were the victims based on the autopsy reports recorded in the morgue of the city of Porto Alegre, Brazil.Materials and methods: Cross-sectional study in which 70 autopsy reports of girls and female adolescents who were killed between January 2010 and December 2016 were analyzed. The cases were evaluated according to the homicide motive or the homicide perpetrator, and five categories were established: drug trafficking related death, femicide, homicide perpetrated by a family member, death preceded by sexual violence, and death related to other transgressions.Results: There was a significant increase in the number of girls and female adolescents who were murdered between 2010 (n=7) and 2016 (n=19). Most of the homicides (64.2%) were related to drug, while femicide occurred in 15.7% of the cases. Homicides perpetrated by a family member, or preceded by sexual violence or related with other transgressions were less frequent as they occurred in 10%, 5.7% and 4.2% of the cases, respectively.Conclusion: Greater attention must be paid to the increase in the number of drug trafficking related homicides among female adolescents when creating and implementing relevant public policies.
